| Objective: To evaluate the clinical efficacy of Chailing Decoction in the treatment of cirrhosis ascites(syndrome of Qi stagnation and water cessation)based on prescription syndrome differentiation.Methods: According to the inclusion criteria,78 patients with Qi-stagnation and water-stopping syndrome who were hospitalized in the Department of Liver Diseases of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine(Guangxi Treatment Center for Liver Diseases of Traditional Chinese Medicine)from January 2020 to December 2020 were selected,and a randomized controlled clinical trial was conducted.Seventyeight patients with cirrhosis with ascites meeting the inclusion criteria were divided into treatment group and control group according to random number table.The control group was treated with conventional western medicine,and the treatment group was additionally treated with Chailing Decoction.The changes of TCM syndrome score,urine volume,body weight,liver function(ALT,AST,TBIL,ALB),coagulation function(PT,PTA),abdominal color ultrasound(portal vein diameter,spleen thickness),ascites B-ultrasound,liver hardness value(LSM)and other indexes in 2 groups before and after treatment were observed and compared.SPSS 26.0 statistical analysis software was used for statistical treatment,and the clinical efficacy of the two groups of patients was compared.1.The comprehensive efficacy of patients after treatment was compared,the total effective rate of the treatment group was 87.18%,the total effective rate of the control group was 74.36%,the difference between the two groups was statistically significant(P < 0.05),the comprehensive efficacy of the treatment group was better than the control group.2.Comparative analysis of the efficacy of TCM syndromes in patients after treatment: the total effective rate in the treatment group was 89.74%,and the total effective rate in the control group was 69.23%.The difference between the two groups was statistically significant(P < 0.05),and the efficacy of TCM syndromes in the treatment group was superior to the control group.3.Efficacy analysis of liver function improvement in patients: after treatment,liver function indexes(ALT,AST,TBIl,ALB)in the two groups were improved compared with before treatment(P < 0.05),and the therapeutic effect in the treatment group was better than that in the control group(P < 0.05).4.Curative effect analysis on the improvement of patients’ coagulation function:after treatment,the coagulation function(PT,PTA)of the two groups were improved compared with that before treatment(P < 0.05),and the therapeutic effect of the treatment group was better than that of the control group(P < 0.05).5.Efficacy analysis of improving portal vein diameter,spleen thickness and liver hardness value: after treatment,liver hardness value of the two groups decreased compared with before treatment(P < 0.05),and the treatment group had better efficacy than the control group(P < 0.05).The portal vein diameter and spleen thickness were not improved in both groups(P>0.05).6.Efficacy analysis of improving patients’ 24-hour urine volume,body weight and ascites grade: after treatment,24-hour urine volume,body weight and ascites grade in the two groups were all improved compared with before treatment(P < 0.05),and the therapeutic effect in the treatment group was better Results:than that in the control group(P < 0.05).7.There were no obvious adverse reactions in the two groups during the treatment period,and no obvious abnormalities were found in vital signs,blood routine,stool routine,kidney function,electrolyte,electrocardiogram and other safety indicators.Conclusions:1.Compared with Western medicine alone,Chailing Decoction has a more obvious effect on the treatment of patients with cirrhosis of Qi-stagnation and water-stopping ascites,it can not only improve the patient’s liver function index(ALT,AST,TBIL,ALB),the function of blood coagulation indexes(PT,PTA),liver hardness value,24-hour urine output,body quality and grade of ascites,etc,and can improve the patient’s clinical symptoms of traditional Chinese medicine,and no significant adverse reactions.2.After clinical research and analysis,the prescription of Chailing Decoction can be preliminarily refined:(1)The abdomen is full and full,and it is like a drum;(2)The two flanks are full;(3)Flank pain wandering erratic;(4)Not to eat and drink;(5)Swollen after eating;(6)Unwell belching;(7)Thirst and poor urination.Tongue and pulse: pale and fat tongue,greasy fur,stringy pulse. |
