Objective:In this study,magnetic resonance imaging was used to evaluate the relationships between correlation values of endplate concavity Angle,MODIC changes and disc height and the volume of lumbar disc herniation in patients with TCM blood stasis constitution.It investigated the changes of vertebral lamina after lumbar disc herniation.It provides important imaging theoretical basis for the diagnosis,treatment and prevention of lumbar disc herniation.This study illustratesd the pathogenesis of lumbar disc herniation caused by intervertebral disc and vertebral endplate.It has more advanced exploration significance in this field.Methods:This study recorded the concava Angle of the upper and lower endplates,MODIC area,sagittal vertebral body area,MODIC area rate,MODIC classification,longest sagittal diameter height,shortest sagittal diameter height,ratio of intervertebral disc height,and volume of lumbar disc herniation in L3/4,L4/5 and L5/S1 segments in the observation group(64 patients with lumbar disc herniation of blood stasis constitution),to analyze the correlation between the volume of lumbar disc herniation and the change of endplate in patients with blood stasis constitution.And comparing the difference of the volume of lumbar disc herniation in different genders and Modic types of blood stasis constitution.It also recorded the concava Angle of the upper and lower endplates,sagittal vertebral body area,longest sagittal diameter height,shortest sagittal diameter height in L3/4,L4/5 and L5/S1 segments in the control group(64 healthy volunteers),it compared the differences of related data between observation group and control group observed the changes of endplate after lumbar disc herniation in patients with blood stasis constitution.Results:In this study,there were 64 people in the observation group,including33 males and 31 females,ranging in age from 30 to 78 years old,with an average of 56.40±11.51 years old.A total of 100 lumbar disc herniations were measured,including 13 cases of L3/4,50 cases of L4/5 and 37 cases of L5/S1.There were 5 cases of Ⅰ type,56 cases of Ⅱ type,3 cases of III type in MODIC classification.The volume of lumbar disc herniation in blood stasis constitution was significantly positively correlated with MODIC area and MODIC area rate,there was a significant negative correlation between the height of the longest sagittal diameter,there was a significant positive correlation with the ratio of disc height,there was no significant correlation with the Angle of concave Angle of upper and lower endplates.There was no significant difference in salient volume between different genders and Modic classifications.there were 64 people In the control group,including 25 males and 39 females,ranging in age from 20 to 45 years old,with an average of 29.45±6.88 years old.The concave Angle of the lower endplate in normal people tends to be flatter than that of the upper endplate,the smallest is L5/S1 upper endplate concave Angle,the largest is L5/S1 lower endplate concave Angle;the highest disc height is L4/5;the largest vertebral body area in sagittal position is L3;The age of observation group was significantly higher than the control group;The lower endplate concave Angle of L3/4 and L4/5 and the upper endplate of L5/S1 in the observation group were higher than those in the control group,and the lower endplate concave Angle of L5/S1 was lower than that in the control group;The height of the herniated disc in L3/4 and L4/5 of the observation group was lower than that of the control group;The sagittal vertebral body area was not statistically significant.Conclusion:(1)The volume of lumbar disc herniation in patients with blood stasis constitution was positively correlated with MODIC area and MODIC area rate.(2)The volume of lumbar disc herniation in patients with blood stasis constitution was negatively correlated with the height of the longest sagittal diameter and positively correlated with the ratio of disc height.(3)The volume of lumbar disc herniation in patients with blood stasis constitution had no significant correlation with the concave Angle of the corresponding upper and lower endplates.(4)The age of patients with lumbar disc herniation of blood stasis constitution was significantly higher than that of the control group.(5)The concave angles of L3/4,L4/5 inferior endplates and L5/S1 upper and lower endplates of patients with lumbar disc herniation with blood stasis constitution tended to be more flat than those of normal subjects.(6)The disc height of L3/4 and L4/5 of patients with lumbar disc herniation of blood stasis constitution was smaller than that of normal people.
