The Clinical Observation Of Tonifying Kidney And Astringent Therapy And Blood-activating Method On Early Diabetic Nephropathy Of Kidney Deficiency And Blood Stasis Type And Its Influence On CysC And HCY

Objective:To observe the effects of tonifying kidney and astringent therapy and blood-activating method on urine microalbumin(mAlb),serum cystatin C(CysC)and homocysteine(Hcy)in patients with early diabetic nephropathy(DN)of kidney deficiency and blood stasis,evaluating the clinical efficacy of tonifying kidney astringent therapy and blood-activating method,clarifying the effectiveness of this method in the treatment of early diabetic nephropathy,and observing its drug safety during the treatment process,hoping to provide new strategies for the treatment of early diabetic nephropathy,and give play to the advantages of traditional Chinese medicine treatment.Methods:Sixty patients with early DN(syndrome of kidney deficiency and blood stasis)from the outpatient and ward of Nanjing Traditional Chinese Medicine Hospital were divided into treatment group 30 cases and control group 30 cases according to the principle of random.Both groups were given conventional therapy(including changes lifestyle,blood sugar control,lipid metabolism disorders,etc.),the control group was given valsartan capsule 80mg once a day on the basis of conventional treatment;the treatment group was given valsartan capsule 80mg once a day and Wuzi Yanzong Decoction on the basis of conventional treatment(Cuscuta 20g,Wolfberry 20g,Schisandra 10g,Rosa laevigata 30g,Semen plantaginis 10g,Hirudo 3g,Notoginseng Root 6g),the course of treatment is 8 weeks.By observing the changes in TCM clinical symptoms,glucose and lipid metabolism indicators,kidney injury indicators,CysC,HCY and other indicators of the treatment group and the control group before and after treatment,the effectiveness of the treatment of this program was evaluated.Results:Through SPSS26.0 software analysis of the results of this experiment,in terms of the efficacy of TCM syndromes,the total effective rate of the treatment group was 96.67%,and the total effective rate of the control group was 46.67%;the treatment group was better than the control group;Both groups had good curative effect in reducing urinary microalbumin,urinary albumin/creatinine ratio and reducing kidney damage,improving glucose metabolism,and the curative effect of treatment group was significantly better than that of control group(P<0.05).Compared with the control group,the treatment group had more significant effect in reducing the levels of CysC and HCY,and the efficacy was better than that of the control group(P<0.05).but there was no significant difference between the two groups in improving blood lipids and renal function(P>0.05).And in the process of clinical research,the two groups of patients had no adverse reactions,and the safety was good.Conclusion:Tonifying kidney and astringent therapy and blood-activating method can effectively improve the clinical symptoms of early DN patients with kidney deficiency and blood stasis,improve glucose metabolism,effectively reduce the patient’s urine protein excretion.At the same time,it can reduce the levels of CysC and HCY,reduce kidney damage,protect the kidneys,and confirmed the clinical efficacy of the method of tonifying kidney astringent therapy and blood-activating method in early DN,laying a foundation for further research on the treatment of early DN with traditional Chinese medicine.
