Objective To explore the diagnostic value of multi-slice spiral CT for atypical liver abscess,and to improve the level of differential diagnosis with intrahepatic cholangiocarcinoma,try to achieve early diagnosis and treatment,and improve the recovery.Methods This study included 23 patients with liver abscesses confirmed by ultrasound puncture drainage and puncture bacterial culture and 25 patients with intrahepatic cholangiocarcinoma confirmed by pathology from January 2018 to October 2020.They were divided into atypical liver abscess group and intrahepatic Cholangiocarcinoma group.Record the general information,laboratory examinations and medical history of the two groups of patients.Observation of CT image content is divided into four parts,which are the inside of the lesion,the edge of the lesion,around the lesion,and other related signs.Among them,the inside of the lesion includes CT values in the third stage of the enhanced scan and blood vessel passage;the edge of the lesion includes wall thickness,mass reduction sign,and delayed uniform enhancement sign;the periphery of the lesion includes transient abnormal enhancement and venous embolism;other related signs include package Membrane depression sign,intrahepatic bile duct dilation and ascites.Results 1.Comparison of the enhanced CT image signs of the lesions of the two groups of patients.There are differences in the CT value,vascular penetration,wall thickness,mass reduction sign,delayed uniform enhancement sign,transient abnormal enhancement,and venous embolism in the three-stage enhanced scan.Statistically significant(P<0.05).2.The general information of the two groups of patients,the difference in age and maximum diameter was statistically significant(P<0.05).3.There were statistically significant differences in diabetes and biliary system infection in the medical history of the two groups(P<0.05).Conclusions Enhanced CT scan can provide effective evaluation for the diagnosis of atypical liver abscess and differentiate it from intrahepatic cholangiocarcinoma.Wall thickness,tumor reduction sign,delayed uniform enhancement sign,venous embolus and transient abnormal enhancement have certain value in the diagnosis of atypical liver abscess.
