Objective:To explore and evaluate the effectiveness and feasibility of acellular dermal matrix(ADM)combined with rabbit autologous auricular cartilage tissue homogenate in repairing auricular cartilage defects in rabbits,and provide a reference for the clinical use of acellular dermal matrix to repair auricular cartilage defects.Methods:1.Establish an animal model for the study of rabbit auricular cartilage defect wound repair and conduct experimental groups.54 New Zealand white rabbits aged 2 to3 months were selected,and a 1.0*1.0 cm size full-thickness ear cartilage defect was made in the central area of the inner side of the right ear of the rabbits.They were randomly divided into 3 groups,with 18 experimental rabbits in each group.Namely group A: cartilage tissue/ADM group;group B: ADM group;group C: blank control group.2.Preparation of rabbit ear cartilage tissue homogenate.3.Treat the rabbits in each group: group A: cartilage tissue/ADM repair,that is,immediate implantation of cartilage tissue/ADM complex into the rabbit auricular cartilage defect wound;group B:ADM repair,simply implantation of ADM;group C : The blank control group only made auricle cartilage defect model,and the skin was sutured in situ.The auricles of the experimental rabbits in each group were sampled at 6 weeks,12 weeks and 18 weeks after the operation,and gross observation and histological observation(HE staining)were performed concurrently.4.According to the Wakitanni Cartilage Defect Repair Score Scale,the scores were scored,and the data obtained were statistically analyzed to evaluate the repair effects of each group.successfully made.There were no deaths in the three groups of experimental rabbits,and all were included in the final analysis.2.Observation of gross specimens showed that in the blank control group,fibrogranulation tissue was repaired at 6 weeks and 12 weeks,and no new cartilage was seen;at 18 weeks,the wound was repaired like a scar.In the ADM group,at 6 weeks and 12 weeks,a small amount of fibrous tissue was formed on the surface of the implanted ADM,without obvious cartilage tissue formation;at 18 weeks,a little new cartilage and some fibrous tissue were formed on the edge of the ADM cartilage defect,and it merged well with the edge of the cartilage defect.A little degradation can be seen.Cartilage tissue/ADM group 6 weeks and 12 weeks showed that ADM and the surrounding cartilage defect wounds were well fused,and thin new cartilage tissue formation was seen on the surface;at 18 weeks,the cartilage donor site wounds were well repaired,and the surrounding normal cartilage tissues were basically integrated.,The new cartilage on the surface is further increased,and the part of the touch can reach a sense of protrusion.3.Histological observation(HE staining)showed that there were no chondrocytes but proliferating fibroblasts in the blank control group at 6 weeks and 12 weeks;at 18 weeks,there was a further increase in proliferating fibroblasts,and the cell arrangement was disorderly.See new chondrocytes.Scattered chondrocyte clusters were seen under the microscope at 6weeks in the ADM group,mainly located at the edge of the cartilage defect area,scattered around and surrounded by collagen fibers;at 12 to 18 weeks,the clumpy chondrocyte aggregation area further increased,and some gathered toward the center,New chondrocytes can be seen in the cell cluster,surrounded by elastic fibers woven into a network.In the cartilage tissue/ADM group,at 6 weeks to 12 weeks,the specimens showed chondrocyte aggregation areas distributed in strips.The chondrocytes were arranged in a regular manner,and the neonatal immature chondrocytes were seen at the edges,and the chondrocytes gradually migrated to the center to mature;it was visible at 18 weeks Multiple mature chondrocyte clusters fuse Results : 1.The experimental model of rabbit ear cartilage defect wound was with each other to form large cartilage cells,which have a typical cartilage structure,and the cartilage matrix around the chondrocytes is synthesized vigorously.4.Wakitanni cartilage defect repair histological score analysis: the repair effect of the ADM group was better than that of the blank control group(P<0.05),and the repair effect of cartilage tissue/ADM group was significantly better than the blank control group(P<0.05).0.01);there was no statistical significance between the cartilage tissue/ADM group and the ADM group at 6 weeks(P>0.05),and there was statistical significance between the cartilage tissue/ADM group and the ADM group at 12 weeks and 18 weeks(P <0.05).Conclusion:ADM can induce the proliferation and regeneration of rabbit auricle elastic chondrocytes.ADM combined with autologous cartilage tissue homogenization can effectively achieve the regeneration and repair of rabbit auricular cartilage defects.ADM implantation alone is also better than self-repair of cartilage defects,providing an economical,safe,convenient and effective cartilage for clinical use A new method of defect repair.
