Clinical Observation Of Refractive Correction Intraocular Lens

Purpose: To compare the visual quality and patient satisfaction of intraocular lens after cataract phacoemulsification combined with regional refractive multifocal intraocular lens implantation and refractive-diffractive multifocal intraocular lens implantation,as well as to evaluate the clinical effect of refractive correction intraocular lens.Methods: This observational research included 57 eyes of 42 cataract patients who underwent the cataract phacoemulsification combined with multifocal intraocular lens implantation in our hospital.According to the different intraocular lenses,the included eyes were divided into two groups: group A(implanted with Lentis Mplus LS-313 MF30 multifocal intraocular lens,26 eyes of 20 cases),group B(implanted with Acry Sof IQ Re STOR + 3D(SN6AD1)multifocal intraocular lens,31 eyes of 22 cases).We determined the uncorrected visual acuity and the best corrected distant visual acuity of these patients during the 3 months followed-up period.Besides,the wavefront aberrations under 4mm pupil of whole eyes,cornea and the intraocular,total ocular modulation transfer function(MTF)as well as total ocular strehl ratio(SR)were also investigated by the VF-14-CN scale questionnaire.Results: 1.There was no significant difference in the uncorrected distant visual acuity,corrected distal visual acuity,uncorrected intermediate visual acuity and uncorrected near visual acuity between group A and group B(P > 0.05).2.Under the pupil diameter of 4mm,there were significant differences in total ocular higher-order aberrations,coma,trefoil and tilt between group A and group B(P < 0.05).However,no significant difference was observed in total ocular spherical aberration between the two groups(P > 0.05).Besides,we observed that there weresignificant differences in intraocular higher-order aberrations,coma,trefoil,tilt and spherical aberration between the two groups(P < 0.05).Nevertheless,there was no significant difference in corneal higher-order aberration,coma,trefoil,tilt and spherical aberration between group A and group B(P > 0.05).3.The total ocular SR and the value of MTF at each spatial frequency in group A were significantly different from those in group B(P < 0 05).4.There were peaks for patients in group A and B both at 0D and-2.5D of the defocus curve.5.The scores of CF-14-CN in group A and group B were 1.43 ± 1.22 and 2.08 ± 1.50,respectively,without significant difference between the two groups(P > 0.05).There was no significant difference of the complete lens removal rate of patients in group A and B(80% in group A vs.81.8% in group B,P > 0.05);There was no significant difference of the rate of reading without glasses in group A and B(100% in group A and 90.9% in group B,P > 0.05).No significant differences were observed in the occurrence rate of optical interference symptoms between the two groups(P > 0.05).Moreover,we have observed that there was no significant difference about the night vision satisfaction of patients in group A and B(95% vs.90.9%,P > 0.05).Conclusion: 1.Both regional refractive multifocal intraocular lens and refractive-diffractive multifocal intraocular lens can successfully regain the visual performance of cataract patients.2.The coma,trefoil and tilt aberration of regional refractive multifocal intraocular lens are greater than those of refractive-diffractive multifocal intraocular lens.3.Both regional refractive multifocal intraocular lens and refractive-diffractive multifocal intraocular lens possess good patient satisfaction.4.The application of refractive correction intraocular lens requires detailed communication with patients before operation.The selection of refractive correction intraocular lens needs to be individualized based on the life styles,occupation and preoperative spherical aberration examination of the patients.
