Objective: To investigate the value of S100 B protein expression,uterine artery pulse index and uterine artery resistance index in pregnant women in predicting preeclampsia individually and jointly by detecting the serum S100 B protein expression,uterine artery pulse index and uterine artery resistance index in pregnant women.Methods: A nested case-control study was adopted in this study.A total of 1000 pregnant women who were registered for obstetric examination in Qingdao Municipal Hospital Department of Obstetrics from September 2018 to January 2020 were collected according to the inclusion criteria.General information of each pregnant woman was registered and followed up.Serum S100 B protein expression was detected by double antibody sandwich enzyme-linked immunosorbent assay(ELISA)at 13-16 weeks,and uterine artery pulse index and uterine artery resistance index were detected by color Doppler ultrasound at 22-24 weeks.Regular follow-up was followed up to 6 weeks after the end of pregnancy.Pregnant women were divided into groups according to the outcome of pregnancy.Pregnant women with preeclampsia were set as the case group(n=37),and pregnant women without pregnancy complications or complications during the follow-up were randomly selected as the control group(n=185)according to the matching ratio of 1:5.The differences of serum S100 B protein expression,uterine artery pulse index and uterine artery resistance index between the two groups were analyzed.Logistic regression analysis and recipient operating characteristic curve(ROC)were used to evaluate the efficacy of S100 B protein,UTA-PI and UTA-RI in predicting preeclampsia alone or in combination with UTA-RI.Results:1.A total of 1000 pregnant women who met the criteria during early pregnancy were included.During the follow-up,6 cases of spontaneous abortion,4cases of induced labor during middle pregnancy,49 cases of follow-up failure due to other uncontrollable factors,and a total of 941 cases were followed up to 6 weeks after the end of pregnancy.A total of 37 cases of preeclampsia and 185 cases of normal control group were included in the clinical study.2.Serum S100 B protein expression in the two groups was detected by ELISA,and the S100 B protein expression in the case group was 0.30±0.15ng/ml,and that in the control group was 0.10±0.04ng/ml.Compared with the two groups,the S100 B protein expression in the case group was significantly higher than that in the control group.The difference was statistically significant(P < 0.05).3.The ultrasonic testing 22-24 weeks of gestation uterine artery pulsation index,case group was 0.77 + /-0.13,the control group was 0.58 + /-0.05;The uterine arterial pulse index of the case group was significantly higher than that of the control group,and the difference was statistically significant(P < 0.05).4.Ultrasonic testing 22-24 weeks of gestation,uterine artery resistance index case group was 0.38 + /-0.10,the control group was 0.15 + /-0.07;The uterine artery resistance index in the case group was significantly higher than that in the control group,with statistical significance(P < 0.05).5.Logistic regression analysis showed that all three were correlated with the occurrence of preeclampsia,and serum S100 B protein,UTA-PI and UTA-RI were positively correlated with the occurrence of preeclampsia.6.S100 B separately predict preeclampsia area under the ROC curve was 0.669(p = 0.035,95% CI: 0.576 0.782);Uterine arterial pulse index alone predicted the area under the ROC curve of preeclampsia(P =0.042,95%CI: 0.371-1.032),and Uterine arterial resistance index alone predicted the area under the ROC curve of preeclampsia(0.655,P =0.021,95%CI: 0.072-0.523).Uterine arterial pulse index has the largest area under the ROC curve for the prediction of preeclampsia.7.The sensitivity and specificity of serum S100 B protein expression combined with uterine artery pulse index and uterine artery resistance index in predicting preeclampsia were higher than those of the single index,and the prediction efficiency of preeclampsia was higher.The combined area under the ROC curve for predicting preeclampsia was 0.870(P =0,95%CI: 0.626-0.906),and the sensitivity and specificity were 87.2% and 80.6%.Conclusion:1.The expression of S100 B protein in pregnant women’s serum,uterine artery pulse index and uterine artery resistance index all have predictive effect on preeclampsia,among which uterine artery pulse index has the highest predictive effect.2.The combination of serum S100 B protein expression in pregnant women with uterine arterial pulse index and uterine arterial resistance index is more effective in predicting preeclampsia than the three alone. |
