Objective:Explore the correlation between the body composition of breast cancer patients and peripheral blood inflammation indexes,to identify high-risk patients and provide new ideas for effective cancer prevention and treatment.Methods:Retrospective analysis the clinical data of newly new diagnosis breast cancer patients who admitted to the First Hospital of Jilin University from May 2013 to July2017.Record the patient’s general information:age、height、weight、body mass index(BMI)、molecular subtype、laboratory biochemical indicators:C reactive protein(CRP)、albumin(ALB)、prealbumin(PA)、neutrophil to lymphocyte ratio(NLR)、platelet to lymphocyte ratio(PLR)、systemic immune inflammation index(SII)etc;Measure visceral fat area(VFA)and skeletal muscle mass(SMM)by bioelectrical impedance technology,and triceps skinfold thickness(TST)with skinfold calipers to representing subcutaneous adipose tissue(SAT).The purpose of this study is to evaluate the correlation between peripheral blood inflammation indexes and body composition based on bioelectrical impedance.Results:1.A total of 252 patients were enrolled in this study.The average age was 52 years old,the age range was 24~76 years old,59.1%were<55 years old,and 40.9%were≥55 years old;The average BMI was 24.94 kg/m~2,and the BMI range was 15.5~35.6kg/m~2,including 5 patients(2%)with BMI<18.5 kg/m~2,100 patients(39.7%)with 18.5≤BMI≤23.9 kg/m~2,and 147 patients(58.3%)with BMI≥24 kg/m~2;According to the stage of breast cancer,235 patients(93.3%)were stage I~Ⅲ,and 17 patients(6.7%)were stageⅣ;According to the molecular subtype of breast cancer,67 patients(26.6%)were Luminal A,104 patients(41.3%)were Luminal B,37 patients(14.7%)were TNBC and 44 patients(17.5%)were HER-2;2.Comparison of different clinical characteristics of breast cancer patients with body composition data.The result shows that,VFI was related to the different age group of breast cancer patients.Among them,patients≥55 years of age had higher VFI(P<0.01);In terms of BMI,the level of VFI、TST、SMM were related to the BMI group of breast cancer patients,and compared with patients with BMI<18.5 kg/m~2,the patients who’s BMI≥24 kg/m~2,the level of VFI(P<0.01),TST(P<0.01)and SMM(P<0.01)were significantly higher.In this study,there was no statistically difference between staging and molecular subtype with VFI、TST and SMM(P all>0.05),but it’s observed that the SMM of TNBC patients has a decreasing trend compared with other molecular subtype of breast cancer patients.3.Comparison of different clinical characteristics of breast cancer patients with inflammation levels.The results showed that,in terms of age,CRP、ALB and PA were related to the age group of breast cancer patients.patients who aged≥55 years old have higher levels of CRP(P<0.05)、ALB(P<0.05)and PA(P<0.05);In terms of BMI,CRP was related to the BMI group of breast cancer patients,among which the BMI was18.5~23.9 kg/m~2,CRP levels in patients were lower than those in BMI<18.5 kg/m~2 and BMI≥24 kg/m~2;In this study,there was no statistically difference between molecular subtype and CRP、ALB、PA、NLR、PLR and SII(P>0.05),but in patients with TNBC molecular subtype,the level of CRP、NLR、PLR and SII have a tendency to increase compared with Luminal A patients.4.Correlation analysis between human body composition and peripheral blood inflammatory indexes of breast cancer patients showed that visceral fat index has a positive correlated with CRP(r=0.261,P<0.01)、ALB(r=0.176,P<0.01)、PA(r=0.248,P<0.01);Skinfold thickness has a positive correlation with ALB(r=0.124,P<0.05)、PA(r=0.158,P<0.05);Skeletal muscle mass has a positive correlated with PA(r=0.132,P<0.05);Conclusion:1.There are differences in VFI、TST and SMM in different BMI groups of breast cancer patients.2.The body composition of breast cancer patients is related to CRP、ALB and PA,and the PA is more representative.3.The study did not find any statistical difference between body composition and NLR,PLR and SII.However,in patients with triple-negative breast cancer,we found that the CRP、NLR、PLR and SII have a tendency to increase compared with Luminal A,and its value deserves further evaluation.
