| Objective:The aims of this study are through the means of clinical history to further understand the risk factors of acute phase of cerebral infarction and it’s recurrent, and to analyze the correlation between serum IMA、CD62p levels among acute phase of cerebral infarction patients. Methods:In this study,128 participants were studied.Among which,98 patients who have cerebral infarction were hospitalized in the Department of Neurology in the second hospital of Jilin University from September 2019 to November 2020.They are acute phase of cerebral infarction group. And the remained 30 patients with healthy examination were selected from physical examination center in this hospital, which is control group.According to the times, acute phase of cerebral infarction group was divided into two small groups: 52 patients occurred the disease firstly;46 are reoccurred patients.According to the degree of neurological dificit(NHISS),the acute phase of cerebral infarction group was divided into three groups:66 cases in light group(<7),22 cases in medium group(7~14)and 10 cases in severe group(>14).First of all,the researcher recorded the historical and physical examination data of the patients in the acute phase of cerebral infarction group in detail, the researcher stored and examined the IMA and CD62p’s concentration of the participants’ vein blood by two-antibody one-step sandwich enzyme-linked immunosorbent assay(ELISA)according to the Kit Operation Instruction strictly. The National Institutes of Health Stroke Scale(NIHSS score)was used to assess patient’s Neurological dificit. Then,to understand the risk factors of occurrence and recurrence of acute cerebral infarction deeply, the researcher analyzed collected clinical baseline data.To analyze the relationships of serum IMA,CD62 p levels between the acute phase of cerebral infarction group and the control group, the serum IMA,CD62 p levels of the two groups were compared. To analyze the relationships of the levels of serum IMA,CD62 p between the first occurent and recurrent groups of acute cerebral infarction, the levels of serum IMA,CD62 p between the first occurent and recurrent groups of acute cerebral infarction were compared. To analyze the relationships of the levels of serum IMA,CD62 p among the groups of the light group, medium group and severe group, the levels of serum IMA,CD62 p in each group were compared as well.Above all, this study tries to get further understanding of the risk factors of the acute phase of cerebral infarction’s occurence and recurrence, thus, providing guidance for the secondary prevention of cerebral infarction, and providing help for the diagnosis, treatment and prognosis of acute phase of cerebral infarction from the perspective of molecular biology.Results:(1)The comparison of baseline data among groups Among the acute phase of cerebral infarction group and the control group, the first group and the recurrent group, there is no ststistically different(P>0.05)in terms of age,sex by comparing the the background of clinical data through the examination of c 2 test or Fisher exact probability, classification variables are represented by the number of cases(%).Specifically, the first group and the recurrent group there is statistically significant difference in terms of hypertension, diabetes, heart disease, hyperlipemia(high cholesterol, high triglycerides, elevated low density lipoprotein),smoking, alcohol consumption, high homocysteine and other aspects(P<0.05).(2)The Correlation of risk factors between the first group and the recurrent group of acute phase of cerebral infarction From the method of the multivariate logical analysis, there are two independent risk factors for recurrent acute phase of cerebral infarction, elevated low density lipoprotein(OR=4.416,95%CI=1.464-13.324,P<0.05),hyperhomocysteine(OR=2.923,95% CI=1.036-8.429,P<0.05).(3)The comparison between the acute phase of cerebral infarction group and the control group in terms of IMA and CD62 p,this study shows that the levels of IMA and CD62 p of acute phase of cerebral infarction group are statistically significant higher than the control group(P<0.05).(4)The comparison between the first group and the recurrent group of acute phase of cerebral infarction in terms of IMA and CD62 p levels, this study shows that the IMA and CD62 p levels of the recurrent group are statistically significant higher than the first group(P<0.05).(5)By involving the levels of IMA and CD62 p into the single factor logistic regression analysis, the results showed that levels of serum IMA,CD62 p are the influencing factors of the occurrence and recurrence of acute phase of cerebral infarction(P<0.05).(6)The comparing of the serum IMA,CD62 p levels in light group, medium group, severe group and control group In terms of IMA and CD62 p levels, the result indicates that the levels in light group, medium group, severe group are higher than those in control group(P<0.05),and the difference among per two groups is statistically different(P<0.05).(7)By means of the spearman correlation analysis, this study found that the correlation between the levels of serum IMA(r=0.348,P<0.05),CD62p(r=0.723,P<0.05)and the degree of neurological dificit is positive among patients with acute phase of cerebral infarction.Conclusion:(1)Elevated low density lipoprotein and hyperhomocysteine are the independent risk factors for the recurrence of acute phase of cerebral infarction.(2)The levels of serum IMA,CD62 p are correlated with the degree of neurological dificit(NHISS score)positively.Specifically, the higher the levels of IMA and CD62 p,the worse the condition, the worse the prognosis. |
