Objective: To analyze the factors related to positive surgical margin(PSM)after radical prostatectomy(RP).Methods: The clinical data and data of patients undergoing laparoscopic radical resection of prostate cancer in Jilin Provincial People’s Hospital from 2014 to 2020 were collected,and the complete clinical data and data of 60 patients were finally collected.Patients were divided into two groups according to postoperative pathological margin status: one group was positive margin group,and the other group was negative margin group.The age,prostate volume,serum prostate-specific antigen(preoperative),number of puncture positive needles,percentage of puncture positive needles,Gleason score,Gleason score after radical resection and pathological stage were compared between the two groups.Univariate analysis of the above variables using the statistical method of X2 test showed that P<0.05 was statistically significant.Multivariate Logistic regression analysis was used,and variables with P<0.05 in univariate analysis were included for statistical analysis,and independent risk factors associated with positive margin(PSM)were obtained.SPSS26.0 software was used for statistical analysis of clinical data,and P<0.05 showed statistically significant difference.Results: Among the 60 patients undergoing laparoscopic radical resection of prostate cancer(LRP),positive surgical margins were found in 27 cases and negative ones in 33 cases,with an overall positive rate of 45%.To after the single factor analysis showed that patients with age(P = 0.697),serum prostate specific antigen(P = 0.484),puncture gleason score(P = 0.297),radical surgery specimens gleason score(P = 0.103),prostate volume(P =0.557)and there is no statistical significance between postoperative positive cut edge,said there was no significant correlation between these factors and cut edge positive;The number of puncture positive needles(P<0.001),the percentage of puncture positive needles(P<0.001),pathological T stage(P<0.001)and the positive margin after radical resection were statistically different,suggesting that these factors were correlated with the positive margin after radical resection.The variables of P<0.05 in univariate analysis were included in multivariate logistic regression analysis,and the results showed that the percentage of puncture positive needles(OR=0.074,P=0.049)and pathological stage(OR=0.011,P=0.023)were independent risk factors for PSM after LRP.Conclusion:(1)Independent risk factors for PSM after RP were: the percentage of positive puncture needles before surgery and postoperative pathological stage;(2)Risk factors for PSM after RP were: the number of positive puncture needles before surgery,the percentage of positive puncture needles and postoperative pathological stage;(3)There was no significant correlation between PSA and PSM after RP operation,including preoperative serum PSA,Gleason score of puncture specimen and Gleason score of radical resection specimen.
