Objective: To evaluate the difference of local and local compatibility of acupuncture in the treatment of primary dysmenorrhea patients with cold coagulation and blood stasis,and to provide ideas for the optimization of acupoint compatibility.Methods: patients with primary dysmenorrhea of cold coagulation and blood stasis type were randomly divided into local and distal compatibility group and local compatibility group by random number table method.Guanyuan(CV4),Zhongji(CV3)and bilateral Sanyinjiao(SP6)acupoints were selected in the local and distant compatibility group,and Guanyuan(CV4),Zhongji(CV3)and bilateral Zigong(EX-CA1)acupoints were selected in the local compatibility group.After 3 courses of acupuncture,the patients were followed up in the third menstrual cycle.All patients were evaluated by visual analog scale(VAS),the Cox menstrual symptom scale(COX),traditional Chinese medicine syndrome score and infrared thermal imaging before treatment,after each course of treatment and during follow-up.Results:1.Intra-group comparison: during treatment and follow-up,VAS,the Cox Menstrual Symptom Scale,traditional Chinese medicine syndrome score and infrared thermal imaging of patients in 2 groups were significantly improved compared with before treatment(P<0.05).2.Comparison among VAS groups showed that the distal compatibility group was lower than the local compatibility group at each stage,and the difference was not statistically significant(P>0.05).3.the Cox Menstrual Symptom Scale rating between groups showed that after 1 course of treatment,there was no significant difference in dysmenorrhea symptom score between the two groups(P>0.05).After 2 courses of treatment and follow-up,the symptom rating branch of distal compatibility group was better than local compatibility group(P<0.05).4.Comparison of traditional Chinese medicine syndrome score between the two groups showed that there was no significant difference between the two groups in the first 2 courses of treatment(P>0.05).In the last 1 course of treatment and follow-up,the local distal compatibility group was better than the local compatibility group(P<0.05).5.Comparison between the infrared heat map groups showed that there was no significant difference between the two groups after the first 2 courses of treatment(P>0.05).At the last 1 course of treatment and follow-up,the local distal compatibility group was superior to the local compatibility group(P<0.05).6.In summary,the clinical total effective rate was 87.8% in the local distal compatibility group and 81.3% in the local compatibility group,and the difference was not statistically significant(P>0.05).Conclusion: 1.Both local and distal compatibility and local compatibility are effective in the treatment of primary dysmenorrhea of cold coagulation and blood stasis type.The overall effect is similar,but the long-term effect of local and distal compatibility is better.2.Local and distal compatibility has the same effect as local compatibility in improving pain,and is superior to local compatibility in dysmenorrhea accompanied symptoms and traditional Chinese medicine syndromes.3.The infrared thermal imaging showed that the temperature change was related to the severity of clinical symptoms,which preliminarily indicated that the infrared thermal imaging has certain clinical significance in the evaluation of the curative effect of primary dysmenorrhea. |
