Objective:To study the relationship between thromboelastogram and the progression of liver cirrhosis and clinical indexes.methods:1.collected 93 cases of cirrhosis without portal vein thrombosis and 41 cases of cirrhosis with portal vein thrombosis from January2019 to January 2020 in the Department of infection of the first affiliated Hospital of Xinjiang Medical University.Abdominal ultrasound and abdominal CTA were diagnosed.2.thromboelastogram of patients with cirrhosis :1)comparative differences and correlation analysis based on CTP grade,MELD score,2)correlation analysis of thromboelastogram and conventional coagulation index and other clinical indicators in patients with cirrhosis,3)ordered logistics analysis of patients with mild,moderate and severe cirrhosis according to CTP grade;4)correlation analysis of other indicators affecting cirrhosis.3.data analysis and processing using SPSS26.0 and Graph Prism8software;single factor analysis using independent sample t test,man-wittney test comparison differences,disease progression factors analysis using multi-factor ordered logistics regression analysis,spearman、Pearson correlation analysis;using Graph Prism8 mapping to describe its correlation scatter trend map.Findings Conclusions:1.There was no significant difference between 1.TEG and portal vein thrombosis in cirrhosis(P>0.05),It shows that thromboelastography can not predict portal vein thrombosis in cirrhosis,It’s just a function of coagulation,MA can reflect platelet function(r=0.63,P=0.00),And has a good correlation with the conventional coagulation index,The two can be combined to make a comprehensive judgment.But TEG can reflect the progression of disease in cirrhotic patients with portal vein thrombosis,Orderly logistics shows K values extended,a and MA reduction,All indicated that the coagulation function decreased;Among other clinical indicators,Total bilirubin,D-D dimer,INR,The decrease of albumin indicates disease progression;2.as liver cirrhosis progresses,CTP grades and MELD scores increased,TEG change,The main manifestations were the prolongation of R and K values(P<0.05),and decreased a and MA(P<0.05),There was a good correlation between TEG and MELD、CTP classification(P<0.05).Among them,K was positively correlated with MELD(r=0.37,P=0.00),a negatively correlated with MELD(r=-0.29,P=0.006),MA was negatively correlated with MELD(r=-0.46,P=0.00);Thrombosis group,MELD was positively correlated with K(r=0.48,P=0.002);a(r=-0.34,P=0.03),MA negatively correlated(r=-0.49,P=0.001);As the CTP rises,K also increased(P=0.007),a 、 MA decreased(P=0.018,P=0.004);3.Cirrhotic patients with thromboelastogram and conventional coagulation indicators have a good correlation(P<0.05),clinical can be combined with the use of the two,comprehensive judgment;4.other clinical indicators of cirrhotic patients also change significantly with the progression of the disease,When albumin was positively correlated with HDL(r=0.36,P=0.003);Total cholesterol was negatively correlated with K(r=-0.39,P=0.017),Positive correlation with MA(r=0.35,P=0.034).With the increase of MELD and CTP grades,The patient’s body is gradually in hypercoagulability,low protein and other liver function gradually decreased,HDL decreased with increasing MELD(r=-0.43,P=0.007),IL-6 increased(r=0.42,P=0.011).5.Conclusion:1)TEG the value of predicting portal vein thrombosis of liver cirrhosis alone is low,it can be combined with conventional coagulation index to judge synthetically;2)thromboelastogram is significantly different in different stages of cirrhosis,3)thromboelastogram of cirrhotic patients is well correlated with routine coagulation index,MELD、CTP grade and platelet;4)Other indexes of cirrhosis patients are closely related to liver function,lipid metabolism is abnormal,HDL、TC can reflect liver function.
