Clinical Application Of General Anesthesia Combined With 0.5% Ropivacaine Local Block Anesthesia Of Inferior Alveolar Nerve In Resection Of Mandibular Cystic Lesions

Objective: To investigate the effect of GA with 0.5% ropivacaine block anesthesia of IANB on the comfort level after resection of mandibular cystic lesions.Methods:Before anesthesia the anesthesiologists assessed the risk of surgery according to the patients’ general condition and the severity of cystic lesions,and then classified anesthesia grade according to the American Anesthesia Grade Classification(ASA)to select ASA Ⅰ ～ Ⅱlevel 60 patients with mandibular cystic lesion.The patients were randomly divided into two groups: simple GA group and GA with local anesthesia group,30 cases in each group.Simple GA group in intraoperative were not given any local anesthetic,After successful GA,while patients in the group of GA with local anesthesia were given 0.5%ropivacaine and epinephrine mixture for inferior alveolar nerve block anesthesia.the weights of lossing blood intraoperative,heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP)at 30 minutes after surgery,the degree of pain 2,4,8,12 hours after the operation and the incidence of postoperative complications were compared between the two groups.Results: The effect of anesthesia in two groups was satisfactory,the intraoperative blood loss in the group of GA with local anesthesia was significantly less than that in the group of simple GA,the difference was statistically significant(P<0.05).The indexes of HR,SBP and DBP in the combined group of general anesthesia were significantly lower than the simple group of GA at the end of surgery,with statistical significance(P<0.05).The postoperative pain level,cystic cavity infection,local swelling and other complications in the group of GA with local anesthesia were significantly lower than simple group of GA,with statistical significance(P<0.05).Conclusion: GA with0.5% ropivacaine IANB can significantly reduce the factors affecting patients comfort and recovery speed,and It is beneficial to rapid convalescence following surgery recovery and wound healing.Because the inferior alveolar nerve block anesthesia has a certain postoperative analgesic effect,it can significantly reduce the postoperative stress response and maintain stable hemodynamic status.
