The Construction And Application Research Of Relocation Stress Intervention Plan For Elderly In Nursing Homes

Objective1.To construct an intervention plan for relocation stress of the elderly in nursing homes;2.To explore the application effect of the intervention program on the stress response,psychological adjustment and the incidence of occupancy problems of the elderly in nursing homes.Methods1 Construction of relocation stress intervention programFirstly,literature research method was adopted to read and analyze relevant domestic and foreign literatures on relocation stress,to understand the status and influencing factors of relocation stress of the elderly in nursing homes,and analyze the design and implementation of relevant intervention programs,so as to provide a better reference and guidance for the construction of an intervention program on relocation stress of the elderly nursing homes.Based on qualitative interview and stress system model as the theoretical basis,the first draft of the intervention plan for relocation stress of the elderly in nursing homes was constructed.Using expert consultation and pre-test to modify the intervention plan,then the final test draft of the intervention plan was formed.2 Application of relocation stress intervention programIt was convenient to select two nursing homes in Zhengzhou,which were similar in scale,environment,culture and configuration,as research institutions.The elderly who met the inclusion and exclusion criteria were selected to be included in the whole group,and the two institutions were randomly divided into the control group and the intervention group by coin toss method.The control group was given routine check-in nursing,and the intervention group was given relocation stress intervention program.The intervention lasted for 5 weeks.Before intervention,after intervention,and 1month after intervention,stress response questionnaire,nursing home adjustment scale for the elderly in nursing homes were used to evaluate the intervention effect of relocation stress for the elderly in nursing homes.Results1 Construction of relocation stress intervention program.According to literature analysis and 3 themes and 6 subthemes extracted from qualitative interviews,a preliminary draft of relocation stress intervention plan was constructed.The first draft of the intervention plan was revised through expert consultation and pre-test,and the final duration of the intervention plan was determined to be 5 weeks,once a week.The intervention program includes five intervention themes:first acquaintance-collaboration,catharsis-stress reduction,analysis of causes-active adjustment,change of cognition-positive coping,sharing of feelings-summary of gains.2 Application of relocation stress intervention programA total of 80 elderly people in nursing homes were included,with 5 cases lost to follow-up and 75 effective samples,including 37 in the intervention group and 38 in the control group.(1)Comparison of stress response scores between the two groups after interventionAfter the intervention,repeated measure ANOVA of total stress response scores of the elderly in the two groups showed that there were statistically significant differences in the inter-group effect,time effect and interaction effect of total stress response scores(P <0.05).Pairwise comparison of the intervention group at different time points showed statistically significant differences(P <0.05).(2)Comparison of nursing home adjustment scores between the two groups after interventionAfter intervention,the generalized estimation equation analysis of the total scores of psychological adjustment of the elderly in the two groups showed that there were statistically significant differences in the inter-group effect,time effect and interaction effect(P <0.05).Each dimension was analyzed by the generalized estimation equation,which showed that the scores of the dimensions of emotional distress,emotional depression,and residence acceptance had statistical differences among groups,time,and interaction effects.There was no statistical difference in interpersonal relationship between groups and time effect,but there was statistical difference in interaction effect.There was no statistical difference in the effect of feeling at home between groups,while there was statistical difference in the time and interaction effect.ConclusionsThe relocation stress intervention program for the elderly in nursing homes can reduce the stress response and the incidence of occupancy problems,improve the nursing home adjustment level and provide reference for the staff of the institutions to intervene the elderly.
