ObjectiveIn recent years,the restoration of bone defects in the dental implant area with demineralized human dentin matrix(DHDM)and the increase in the vertical bone height of the alveolar bone have become a research focus in the field of oral implantation.In this experiment,a custom-made titanium container was used to fix the DHDM to the rabbit skull to explore the effect on the vertical bone height growth of the rabbit skull,and to provide basis and reference for the application of the DHDM to the vertical bone augmentation surgery in the oral implant area.Methods1.The design of the titanium container: The pure titanium plate is processed into a titanium container using a CNC confidential machine tool,and it is sterilized after being cleaned and sealed.2.The preparation method of DHDM: Collect the isolated teeth and prepare them into granules with a size of about 0.25-1mm.Use Vacua Sonic vacuum ultrasonic tooth decalcification accelerator for demineralization,and seal it for use after preparation.3.Animal experiment: 8 rabbit vertical bone augmentation animal models were established,titanium containers were fixed on both sides of the midline of rabbit skulls and randomly implanted in the following four groups: Group A was implanted with DHDM combined with autogenous bone(volume ratio: 3:1);Group B was implanted with DHDM;Group C was implanted with calcined bovine bone;Group D was implanted without any material,only blood penetration.4.Observation method: Scanning Electron Microscope(SEM)to observe the morphology of the material.The animals were sacrificed at 1 month and 3months after the operation,and the specimens were taken for gross observation,Micro-CT(Micro Computed Tomography)examination,histomorphology and histomorphometric observation,and then SPSS 17.0 software was used for statistical analysis.Results1.SEM results showed that the dentin tubules on the surface of DHDM were completely exposed.2.The general observation showed that the titanium container was stable without loosening.After the titanium container was removed,the vertical bone height of the rabbit skull was increased in all four groups.3.Micro-CT examination of the bone augmentation area showed that new bone tissue was formed in the four groups.In March,the image of the new bone was larger than that in January.The bone graft materials of the three groups A,B,and C were replaced by the new bone,and only a small amount of material remained in the middle and upper part of the bone augmentation area.4.Observation of the hard tissue section showed that the new bone grew vertically upward from the rabbit skull.In March,the three groups A,B,and C found signs of graft material absorption in the entire graft area.Compared with January,the lower part of the graft area was More mature bone was observed in the upper half.The retention rate of residual material was calculated by morphometry,and there was a significant difference between the January group and the March group(P<0.05).Conclusions1.The new titanium container plays an important role in the increase of the vertical bone height of the rabbit skull,and provides a stable and reliable space support for the increase of the vertical bone height.2.The DHDM treated with compound acid combined with ultrasound has good biocompatibility and shows good osteoconductivity and osteoinductivity in rabbit skull repair.DHDM is expected to be a treatment for clinicians a better choice for patients with insufficient vertical bone mass.3.The quality of new bone formation with DHDM combined with autologous bone is better than that of purely using DHDM.4.The combined application of the new titanium container and DHDM can achieve a reliable increase in the vertical bone height of the rabbit skull.
