Objective:To investigate the clinical feature,risk factors,treatment and prognosis in patients with in-hospital ischemic stroke.Methods: Eighty-five patients with IHS were sequentially recruited in the period 2015.1–2020.1.Their characteristics,care,and outcomes were compared with 170 patients who had an out-of-hospital ischemic stroke(OHS)admitted to the hospital of Xinjiang Medical University.According to the clinical and laboratory data of 2 group,the risk factors that may be related to IHS were screened out and the independent risk factors affecting IHS were further analyzed by binary logistic regression analysis.According to the m RS scores of 0-2 and 3-6,the patients were divided into the group with good prognosis and the group with poor prognosis.The possible risk factors were analyzed by single factor analysis according to their clinical data,and the independent influencing factors were further analyzed by multi-factor regression analysis.Results: The proportions of atrial fibrillation,cardiac dysfunction,renal insufficiency,fever,cancer,perioperative in the IHS group were significantly higher than those in the OHS group.Binary logistic analysis showed that gender(OR=2.921,95%CI1.252~6.815;P=0.013),cardiac dysfunction(OR=4.133,95% CI 1.374~12.439,P=0.012),renal insufficiency(OR=5.196,95% CI 1.370~19.705,P=0.015),cancer(OR=4.551,95%CI 1.188~17.441,P=0.027),m RS scores(OR=2.562,95% CI 1.716~3.825,P<0.001),WBC(OR=1.185,95% CI 1.063~1.322,P=0.002),Hb(OR=0.097,95% CI 0.962~0.995,P=0.012)and Fib(OR=2.181,95% CI 1.554~3.059,P<0.001)were independent risk factors for IHS.Multivariate logistic regression analysis showed that cancer(OR=14.092,CI=1.213~163.702,P=0.034),high m RS score(OR=10.363,95% CI 4.936~21.759,P<0.001),IHS(OR=80.316,95% CI 9.212~700.255,P<0.001),ACS+PCS(OR=35.573,95% CI3.330~379.994,P=0.003)were the independent risk factors for prognosis.Conclusion:Multiple risk factors are presented in the in-hospital stroke,patients with IHS had poorer outcomes than those with OHS.Therefore,the identification and early treatment of in-hospital stroke is particularly important.
