Factors Influencing The Development,Prognosis And Survival Of Brain Metastasis In Breast Cancer

Purpose:To analyze the clinicopathologic data of patients with breast cancer brain metastases,and explore the related factors that affect the development and prognosis of breast cancer brain metastases,so as to better guide clinical treatment.Method:The clinicopathologic data of 80 breast cancer patients with brain metastases in the Fifth Affiliated Hospital of Zhengzhou University January 1,2010 to December31,2018 were collected.The factors that affect the development time of brain metastases,survival time after brain metastases,and overall survival time were analyzed.The SPSS 22.0 software package was used for statistical analysis of the data.Kaplan-Meier method was used for survival analysis,and Cox proportional hazard regression model was used for multivariate analysis.Results:1.The development time of brain metastases in 80 patients was 0-150.80 months,and the median time was 28.20 months.Univariate analysis showed that TNM staging and molecular classification of breast cancer affect the development time of brain metastases(P<0.05).In the multivariate analysis,TNM staging and molecular typing were all statistically significant(P<0.05),which were independent factors influencing the development time of brain metastases.2.The survival time of the whole group of patients after brain metastasis was0.50 months-60.00 months,the median survival time was 9.90 months,the 1-year survival rate was 43.7%,and the 2-year survival rate was 23.7%.Univariate analysis showed that PS score,treatment after brain metastasis and molecular typing affect the survival time after brain metastasis(P<0.05).In the multivariate analysis,the above factors were statistically significant(P<0.05),and they were independent factors influencing survival time after brain metastasis.3.The overall survival time of patients was 8.40-195.80 months,and the median survival time was 39.00 months.Univariate analysis showed that PS score,TNM staging,treatment after brain metastasis and molecular classification affect the overall survival time of patients(P<0.05).In the multivariate analysis,PS score,TNM staging and molecular classification were statistically significant(P<0.05),and they were independent factors of overall survival time.Conclusions:1.TNM stage and molecular typing are independent factors of the development time of brain metastasis.Patients with early TNM stage and Luminal type have later brain metastasis.2.PS score,treatment after brain metastasis and molecular typing are independent factors of survival time.The prognosis of patients with PS score ≤ 1 and Luminal type is better.Active treatment should be taken to improve survival after brain metastasis.3.PS score,TNM stage,molecular classification are independent influencing factors of overall survival time.The overall survival time of patients with PS score ≥2,TNM stage later and non-Luminal type have shorter overall survival time.
