Comparative Efficacy Of Chinese Patent Medicine Combined With Western Medicine In Patients With Vascular Dementia

Objective: In this study,the method of network meta-analysis was used to compare the clinical efficacy of different oral Chinese patent medicines combined with western medicines in the treatment of vascular dementia,and the included interventions were ranked in order to provide evidence support for clinicians to choose drugs for the treatment of vascular dementia.Methods: Use the computer to search the four Chinese databases of China Journal Full-text Database(CNKI),China Biomedical Literature Database(CBM),VIP Chinese Biomedical Journal Database(CQVIP),Wanfang Data Knowledge Service Platform(Wanfang Data)and Pubmed and The Cochrane Library,Web of Science,and Embase.The start and end dates of the search are from the beginning of the database’s establishment of the collection of journal documents to February 1,2021.The collection of relevant clinical randomized controlled trials of Chinese patent medicine combined with western medicine in the treatment of vascular dementia Two researchers independently screened and extracted data related to the literature on the treatment of vascular dementia with Chinese patent medicine combined with western medicine according to the pre-established inclusion and exclusion criteria.According to the research quality and bias risk assessment tools recommended by Cochrane System Review Manual 5.0.1 The quality of the included literature was evaluated,and then Rev Man 5.4 and ADDIS 1.16.7 were used to perform traditional Meta analysis and network Meta analysis on the included literature.Results: According to the established search strategy,a total of 15786 related documents were retrieved and 41 articles were finally included.A total of 3631 patients with vascular dementia were included,including 1831 cases in the experimental group and 1,800 cases in the control group,involving 8 kinds of oral Chinese patent medicines,Including Ginkgo Leaf(or Ginkgo Leaf Capsule),Naomaitai Capsule,Naoxintong Capsule,Tianzhi Granule,Yangxue Qingnao Granule,Fufang Haishe Capsule,Yindan Xinnaotong Soft Capsule and Dengzhan Shengmai Capsule.The results of traditional Meta analysis of the included literature show that oral Chinese patent medicine combined with western medicine can improve the total clinical effective rate of patients with vascular dementia,and the improvement of the scores of patients on the MMSE scale,Mo CA scale and HDS scale is better than that of Western medicine alone.Group,the difference was statistically significant(P ﹤ 0.05);the incidence of adverse reactions between the oral Chinese patent medicine combined with Western medicine group and the Western medicine group alone was not statistically significant(P﹥0.05),the literature reports included in this study No serious adverse events occurred during the observation.The results of network meta-analysis of the included literature show that:1.Clinical effectiveness: Compared with pure western medicine treatment,Fufang Haishe Capsules,Naomaitai Capsules,Naoxintong Capsules,Tianzhi Granules,Qingxue Qingnao Granules,Yindan Xinnaotong Soft Capsules,Ginkgo Leaf combined with Western medicine to treat vascularity The clinical effective rate of dementia was significantly improved(P<0.05),and Yindan Naoxintong soft capsule combined with western medicine ranked the highest.2.Simple intelligence check scale score: Compared with simple western medicine treatment,Dengzhan Shengmai Capsule,Fufang Haishe Capsule,Naomaitai Capsule,Naoxintong Capsule,Tianzhi Granule,Yangxue Qingnao Granule,Yindanxintong Soft Capsule,Ginkgo biloba combined with western medicine significantly improved the patient’s MMSE scale score(P<0.05),and the Dengzhan Shengmai capsule combined with western medicine was superior to Fufang Haishe Capsule,Naomaitai Capsule,Tianzhi Granule,Yangxue Qingnao Granule,Ginkgo biloba combined western medicine scheme;Yindanxintong soft capsule combined with western medicine is better than ginkgo leaf combined with western medicine(P<0.05),and Yindanxintong soft capsule combined with western medicine has the highest ranking.3.Montreal Cognitive Assessment Scale score: Compared with the treatment of vascular dementia with western medicine alone,Naoxintong capsule combined with western medicine significantly improved the patients’ Mo CA scale score(P<0.05).Each oral Chinese patent medicine combined with western medicine was used to treat vascular dementia.Compared with each other,there was no statistical difference in the improvement of the scores of the Mo CA scale,and the combination of Naoxintong capsule and western medicine was ranked the highest.4.Hasegawa Dementia Scale Score: Compared with Western medicine alone,Naomaitai Capsule,Yangxue Qingnao Granules,Ginkgo Leaf combined with Western medicine significantly improved the HDS scale score(P<0.05),and the combination of Naomaitai Capsule and Western medicine was superior Yu Yangxue Qingnao Granules combined with Western medicine program(P<0.05),and Yangxue Qingnao Granules combined with Western medicine program ranked the highest.Conclusion: The results of this system evaluation suggest that in the process of treating vascular dementia,oral Chinese patent medicine combined with western medicine can improve the efficacy and improve the cognitive function of patients.It is also better than pure western medicine.Among them,Yindan Naoxintong soft capsule,Naoxintong capsule,Yangxue Qingnao Granules may be the best choice for combination medication.
