| Objective:In patients with gestational diabetes as the research object of this research,based on the principle of evidence-based medicine and by the constitution and the relevant scale,the TCM syndrome types in patients with gestational diabetes,physical investigation,the distribution characteristics of TCM syndrome type and physique,so as to clinical from the TCM syndrome type and physique level provide certain theoretical basis for prevention and treatment of gestational diabetes.Methods:This research according to the principle of evidence-based medicine in January 2020 to January 2021 collected clinic in affiliated hospital of shanxi medical university during the period of patients with gestational diabetes,accord with standard of research at the same time,use for good general information questionnaire to collect basic information,with the aid of the gestational diabetes syndrome questionnaire to collect syndrome and TCM syndrome factor calculation,and syndrome factor differentiation based on the observation of Chinese medicine constitution classification and decision standard of gestational diabetes patients health,after good clinical information collection,unified by use of Excel software to establish a database for data management,SPSS25.0statistical software was further used for statistical analysis to explore the distribution characteristics of TCM syndrome types and constitutions in patients with gestational diabetes.Results:1 Factors associated with gestational diabetes mellitus1.1 the ageThere are a total of 80 patients with gestational diabetes in this survey.The age is in accordance with the normal distribution,with an average of 31.00±4.06.The oldest is 41 years old and the youngest is 21 years old.Among them,29 people are in the age range of20-30 years old,accounting for 36.25%;50 people aged 30-40,accounting for 62.50%;1person aged 40 and above,accounting for 1.25%.1.2 FertilityIn this survey,the number of pregnancies ranged from 1 to 6,among which the number of first pregnancy was the largest,accounting for 55.00%.The number of deliveries ranged from 0 to 2,of which 1 delivery accounted for 50.00%.Among the modes of delivery,more people were delivered via vaginal delivery,accounting for 71.43%.The number of abortion varies from 0 to 4 times,among which the number of people without abortion is the largest,accounting for 60.00%.In the group with 1 abortion,the number of people choosing induced abortion is more,accounting for 61.90%.1.3 Body mass index(BMI)before pregnancyThe population with normal body mass index was the largest,accounting for 61.25% of the total population,the population with overweight accounted for 22.50%,the population with obesity accounted for 15.00%,and the population with low body mass index accounted for 1.25%.1.4 Family history of diabetesThere were 23 patients with a family history of diabetes,accounting for 28.75%;There were 57 patients without diabetes history,accounting for 71.25%.1.5 History of polycystic ovary12 patients had a history of polycystic ovary,accounting for 15.00%;68 patients(85.00%)had no history of polycystic ovary.1.6 Dietary addictionThe enrolled patients could be divided into 6 categories of dietary preference,and the number of patients with one preference was the largest,accounting for 67.50%.Unbiased population accounted for 20.00%;People prefering 2 kinds or more accounted for 12.50%.In the group preferring one kind,the number of people preferring spicy and light was the largest,both accounting for 38.89%.2 Physical distributionThere were 44 patients with a single constitution,accounting for 55.00%.12 people(15.00%)had two physiques.There were 8 people with 3 or 4 kinds of constitution,accounting for 10%.There were 3 people with 5 or 6 kinds of constitution,accounting for3.75%.There were 2 patients with 7 kinds of constitution,accounting for 2.5%.Will each of the constitution "is" type and "tendency is" type statistical induction,the constitutional type frequency in descending order: Yin deficiency(34,42.50%)> quality deficiency of mass(24,30.00%)> Yang mass(24,30.00%)> humid mass(22,27.50%)>peaceful mass(22,27.50%)> phlegm wet mass(16,20.00)> qi depression mass(14,17.50%)quality(10,12.50%)> > blood stasis,the great mass(3,3.75%).3 Distribution of syndromes,syndrome elements and syndrome types3.1 Syndrome distributionWith the help of the Gestational Diabetes Syndrome Questive Form,a total of 67 syndromes of patients with Gestational Diabetes were collected from 20 aspects,including spirit,cold and heat,sleep,distool,tongue and pulse,etc.,and there were 46 syndromes with a frequency above 10%,and the top 10 syndromes with high frequency were:Pulse sliding(53,66.30%),thirst(44,55.00%),dreamy(38,47.50%),spontaneous sweating(34,42.50%),fatigue(33,41.30%),hot flushing(32,40.00%),dry throat(30,37.50%),irritability(29,36.30%),amnesia(29,36.30%),drowsiness(28,35.00%).3.2 Distribution of syndrome elementsThe collected syndromes were calculated according to Syndrome Differentiation and Syndrome Differentiation.The calculated syndromes were divided into disease site syndromes and disease site syndromes.Among the disease site syndromes,the first three with high frequency were liver(35,43.80%),kidney(25,31.30%)and spleen(15,18.80%).Among the syndrome elements,the top 6 with high frequency were Yin deficiency(58,72.50%),Qi deficiency(41,51.30%),Yang deficiency(22,27.50%),dampness(21,26.30%),heat(21,26.30%)and Qi stagnation(19,23.80%).Each patient’s disease syndrome factor and disease syndrome factor combination,can draw unlicensed vegetarians have 10 people,only one factor in 7 people,there are two syndrome factor of 16 people,there are three factor of 13 people,there are 4 5 persons syndrome factor with five syndrome factor is 6 people,there are nine six card vegetarians,seven syndrome factor of 7 people,with eight syndrome factor is 6 people,10 syndrome factor is 1.3.3 Distribution of syndrome typesTo each patient’s illness and disease syndrome factor combination as a card type,can draw the following card type: qi and Yin deficiency syndrome,Yin asthenia and internal heat,liver Yin deficiency syndrome,mind kidney Yin deficiency syndrome,Yin deficiency syndrome,spleen deficiency and liver Yin deficiency and blood stasis syndrome,spleen sun be the spirit virtual wet hysteresis heat,liver Yin deficiency syndrome of kidney Yang deficiency syndrome,Yin liver blood deficiency of clamp heat,etc.,the combination type variety,more complex,is not conducive to statistics and induction,so the choice for syndrome factor variables,specimens of patients with Numbers for establishing a database,clustering analysis,draw a card type:Deficiency of both Qi and Yin(24,34.29%),deficiency of both Qi and Yin(17,24.29%),stagnation of liver and Qi(9,12.86%),deficiency of kidney and Yang(8,11.43%),internal heat of Yin deficiency(7,10.00%),dampness of spleen deficiency(5,6.25%).Conclusion:1.The constitutions of patients with gestational diabetes mellitus are diverse,and the three main constitutions are Yin deficiency,Qi deficiency and Yang deficiency.2.The main pathologic elements of gestational diabetes mellitus are liver,kidney and spleen,and the main pathologic elements are Yin deficiency,Qi deficiency,Yang deficiency,dampness,heat and Qi stagnation.3.The main syndrome types of patients with gestational diabetes mellitus are:deficiency of Qi and Yin,deficiency of Kidney and Yin,stagnation of Liver-Qi,deficiency of Kidney and Yang,internal heat of Yin deficiency,and dampness of spleen deficiency. |
