Objective: To explore the differential diagnosis value of CT multi-parameter comprehensive analysis in thoracic exudate and leakage.Methods: Retrospective analysis From January 2018 to August 2018,our hospital performed 30 patients with CT pleural scan and confirmed by histological or biochemical examination of pleural effusion,including 16 cases of exudate and 14 cases of leakage.Using the comprehensive spectrum analysis platform,the conventional mixed energy CT values of various regions of interest(ROI)of each patient’s pleural effusion,CT values of different single energy ke V(40~140 ke V)and slopes of energy curves were measured and calculated.And do the corresponding statistical analysis to analyze whether the two sets of data are different.Results: The conventional 140 ke V mixed energy CT value exudate group [(19.30 ± 3.12)HU] in the exudate group was higher than the CT value of the leakage group [(11.15 ± 1.90)HU],and the data of the two groups were different and statistically significant(t =-7.701,P<0.001);the CT values of the two groups of data at40~140ke V were statistically significant(P<0.05).The single-energy value was 40 ke V,the difference between the two groups was the largest,with the energy value.The difference between the two groups is gradually reduced.The similar trend of the energy spectrum curves of the two groups is the downward curve of the archback;the slope of the energy spectrum curve of the leakage group is 0.32 ± 0.09 lower than that of the exudate group.(0.61±0.24),the data of the two groups were different and statistically significant(t=-3.109,P=0.002);the effective atomic number of the exudate group was(7.83±0.11)and the leakage group was(7.70±0.04).There were significant differences between the two groups(P<0.001).The data of iodine(water)and water(iodine)in the leakage group and the exudate group were statistically significant(P< 0.05),and the data values of each group in the exudate group were higher than the leakage group.Conclusion: The exudate and leakage have different energy spectrum curves and energy spectrum characteristic substances.The energy spectrum CT has certain reference value for the differential diagnosis of thoracic exudate and leakage.
