| Objective:Through a case-control study,we retrospectively analyzed the correlation between color ultrasound Doppler measurement of different ultrasound indexes of fetuses at different gestational weeks and the diagnosis of fetal growth restriction.Methods:1.The clinical data of pregnant women who were hospitalized in the obstetrics department of our hospital from January 2018 to April 2019and terminated pregnancy at≥28 weeks and≤42 weeks were retrospectively collected.2.In the propensity score,87 pregnant women with fetal growth restriction were selected as FGR group,and the other 87 control group as NFGR group.3.Collect fetal BPD(fetal head double top diameter),HC(head circumference),AC(abdominal circumference),FL(femur length),HL(humeral length),RI(resistance index),PI(Pulsation Index),S/D(the ratio of fetal umbilical artery systolic pressure to diastolic pressure).4.Compare the detection indexes of the two groups of fetuses,and analyze the correlation between ultrasound index data and fetal growth restriction.Results:1.The difference between the two groups of gestational age,BMI,pregnancy weight gain,and occupation was statistically significant(P<0.05).2.The incidence of adverse pregnancy outcomes in the FGR group was higher than that in the NFGR group.3.Comparison of the results of different ultrasound indexes of FGR group and NFGR group at the same gestational week.3.1 There was no statistically significant difference between the FGR group and the NFGR group at 14 to 19+6weeks of ultrasound measurement of fetal growth indexes,uterine artery and blood flow indexes(P>0.05).3.2 Ultrasound at 20 to 23+6weeks:Compared with the NFGR group,the fetal FL in the FGR group decreased(P=0.014),and the S/D increased(P=0.028).The area under the ROC curve of FL is 0.688,the cutoff value is 35.7 mm,the sensitivity is 0.536,and the specificity is0.792.3.3 From 24 to 27+6weeks of ultrasound:Compared with the NFGR group,the fetal HC,AC,FL,and HL in the FGR group decreased(P<0.05).The area under the curve of HL is 0.762,the cutoff value is 43.95mm,the sensitivity is 0.683,and the specificity is 0.736.3.4 From 28 to 31+6weeks of ultrasound:Compared with the NFGR group,fetal BPD,HC,AC,FL,HL decreased(P<0.001),RI,PI,S/D increased(P<0.05).The areas under the ROC curve of FL and HL are0.711 and 0.807,the cutoff values are 54.5mm and 49.5mm,the sensitivity is 0.851 and 0.915,and the specificity is 0.49 and 0.608,respectively.3.5 From 32 to 35+6weeks of ultrasound:Compared with the NFGR group,the fetal BPD,HC,AC,FL,and HL of the FGR group decreased(P<0.01),and the PI increased(P<0.05).The area under the ROC curve of HL is 0.803,the cutoff value:55.5 mm,the sensitivity is 0.716,and the specificity is 0.734.3.6 Ultrasound at 36 to 37+6weeks:Compared with NFGR group,fetal BPD,HC,AC,FL,HL in FGR group decreased(P<0.001).The area under the ROC curve of AC is 0.837,the cutoff value is 307.5mm,the sensitivity is 0.87,and the specificity is 0.705..3.7 From 38 to 39+6weeks of ultrasound:Compared with the NFGR group,the fetal BPD,HC,AC,FL,and HL of the FGR group were reduced(P<0.001).The area under the ROC curve of FGR predicted by AC and HL is 0.871 and 0.847,the cutoff values are 306.5mm and61.5mm,the sensitivity is 1,0.654,and the specificity is 0.577 and 0.855,respectively.CONCLUSIONS:1 The incidence of adverse pregnancy in the FGR group is higher than that in the control group;2 Ultrasound indicators in early pregnancy have little value in predicting fetal growth restriction;3 HL is highly predictive of FGR from 28 to 35+6weeks of pregnancy;4 AC 36~39+6weeks gestation is more effective in predicting the occurrence of FGR... |
