ObjectiveTo evaluate whether Chinese version of Identify Chronic Migraine(ID-CM)can be used as a screening tool for chronic migraine in China,and further explore the influence factors of chronic migraine(CM).MethodsIn this study,the headache questionnaire was powered by www.wjx.cn to collect people who completed the questionnaire from November 22,2019 to January 31,2020.Headache questionnaire included Chinese version ID-CM,General information questionnaire,Patient Health Questionnaire-4(PHQ-4)and Athens Insomnia Scale(AIS).The original ID-CM was translated into Chinese by the researchers,and the Chinese version of ID-CM was finalized after discussion and preliminary experiments,the questionnaire diagnoses are divided into probable chronic migraine(PCM)and non probable chronic migraine(non PCM).A question about the difficulty of Chinese version of ID-CM was added to assess the patient’s understanding of the questionnaire;General information questionnaire include general information and headache characteristics;PHQ-4 and AIS were used to assess anxiety,depression and insomnia.Neurologists conducted clinical diagnosis of the investigated population by telephone or in person within 2 weeks.Diagnosis can be divided into CM,episodic migraine(EM),chronic tension-type headache(CTTH),other headache and non headache.In this study,Kappa test was used to test the consistency of Chinese version of ID-CM and clinical diagnosis and carried out univariate and Logistic multivariate analysis on the influencing factors of CM.ResultsIn this study,532 headache questionnaires,27 PCM and 24 CM questionnaires were included.(1)The sensitivity and specificity of Chinese version ID-CM were 87.50% and 98.82% respectively.The positive predictive value was 77.78% and the negative predictive value was 99.41.The Kappa coefficient of the questionnaire and the clinical diagnosis was0.814(95%CI: 0.694~0.934,P<0.001),showing high consistency.(2)Compared with the non-headache group,there was a statistical difference in the prevalence of CM among people of different ages,marital status,educational level,night shift frequency,anxiety,depression and sleep condition(P<0.05).Logistic multivariate analysis showed that married,illiteracy-primary school education level and insomnia were independent risk factors for CM.(3)Compared with EM population,CM population had the highest incidence of illiteracy-primary school education level,and the lowest risk of CM in people with master’s degree or above.There was a statistical difference between CM and CTTH in different exercise frequencies.(4)Further assessment of risk factors for anxiety,depression and insomnia in the CM population,the risk factors for anxiety tendency were severe labor intensity and depression;the risk factors for depression were anxiety;no risk factors for sleep disorder was identified.Conclusions1.The Chinese version of ID-CM is a simple and accurate screening tool,which can diagnose most CM patients in the general population,and patients with headache have a high degree of acceptance of this questionnaire,which is suitable for clinical promotion.2.In this sample,people who are married,have a illiteracy-primary education level,are anxious,and have sleep disorders have a higher risk of CM.The risk of chronic migraine is illiteracy-primary school level.3.Further analysis found that in the CM population,anxiety and depression affected each other,and heavy labor intensity was significantlyassociated with an increased risk of anxiety.
