Hypertensive disorders complicating pregnancy(HDCP)is a group of diseases that coexist with gestational hypertension and seriously threat maternal and fetal health.Pregnancy is a very special period for women,and hypertension,a systemic disease,is not only harmful to the health of pregnant women,but also to the fetuses,HDCP is not only harm their health,also have negative effects on the fetus,the author intends to use ultrasound parameters are relatively comprehensive and systematic evaluation of late pregnancy gestational hypertension disease cardiac function and fetal growth and development,close monitoring of pregnancy,to avoid the occurrence of adverse clinical outcomes.In this study,150 pregnant women aged 24 to 34 and gestational weeks 28 to 41+6 weeks were selected.All menstrual cycles were regular,the last menstrual period was clear,no systemic diseases,singleton pregnancy,and excluded maternal and fetal cardiovascular abnormalities.Among them,60 cases of pregnant women with HDCP were in the disease group and 90 cases of pregnant women with normal blood pressure were in the control group.There was no significant difference in age,body mass and other general materials between the two groups(P > 0.05).The ultrasonic parameters of the two groups of pregnant women were measured as follows: LVDd,LAD,IVSTd,LVPWTd,LVEF,TDI was used to measure the Ea peak and Aa peak of the velocity of the anterior lobe of the mitral ring,and the Ea/Aa value of the velocity of the anterior lobe of the mitral ring was calculated;To obtain and measure the motion spectrum atthe junction between the anterior tricuspid valve and the annulus of the two groups of fetuses,ICT,IRT,ET were measured,and Tei index of right ventricle was calculated;Fetal growth indicators,BPD,HC,AC,FL,were measured on standard sections.SPSS 23.0 was used to compare and analyze the differences between the two groups.The measurement data were expressed as mean ± standard deviation(),the measurement data between the two groups were compared by independent sample t test,the counting data were tested by c2 test,and the correlation analysis was conducted by Pearson correlation analysis,with P < 0.05 as the difference was statistically significant.The results are as follows:1.Compared with the control group,pregnant women in the disease group showed increased LVDd and LAD,and increased IVSTd and LVPWTd,with statistically significant differences(P< 0.05).2.The decrease of left ventricular diastolic function in pregnant women in the disease group was statistically significant compared with that in the control group(P< 0.05).3.No cases of decreased left ventricular systolic function were collected in the disease group and the control group,and the difference was not statistically significant(P> 0.05).4.The Tei index of the right ventricle of the fetus in the disease group was higher than that in the control group,and the difference was statistically significant(P< 0.05).5.In the disease group,fetal right ventricular Tei index was positively correlated with maternal blood pressure,and the difference was statistically significant(r= 0.605,P< 0.05).6.Fetal BPD,HC,AC and FL in the disease group were smaller than those in the control group,and the differences were statistically significant(P< 0.05).Pregnant women in the disease group showed increased LVDd and LAD,increased IVSTd and LVPWTd,and decreased left ventricular diastolic function,providing imaging basis for clinical intervention of blood pressure in pregnant women with HDCP.Fetal right ventricular disease group Tei index is higher than the control group,shows that HDCP pregnant women not only damage the heart,also to a certain extent affect the function of the fetal heart;In addition,the results suggested that the Tei index of the right ventricle of the fetus was positively correlated with the blood pressure of HDCP pregnant women,which also suggested that more attention should be paid to the control of blood pressure of pregnant women.The measured values of BPD,HC,AC and FL in the sick group were smaller than those in the control group,which provided valuable information for clinical selection of delivery timing and delivery mode.To sum up,through the measurement and analysis of ultrasonic parameters of maternal fetal heart function and fetal growth and development in late pregnancy of hypertensive disease during pregnancy,it can provide valuable information for integrated management of clinical mother and fetus and multidisciplinary cooperation,so as to achieve a good pregnancy outcome.
