BackgroundIntracranial aneurysms have a high incidence rate in Chinese population.The rupture of the tumor may lead to intracranial hemorrhage,which leads to high morbidity and mortality.Later treatment and recovery also bring heavy burden to family and society.The key points of the clinical concern were always been the way to find,diagnose and treat as soon as possible.At present,the commonly used methods include digital subtraction angiography(DSA),multislice spiral CT angiography(MSCTA)and magnetic resonance imaging(MRI).DSA has a high accuracy rate in the diagnosis of intracranial aneurysms(the accuracy and sensitivity rate are up to 99%),which is generally recognized as the gold standard for the examination of intracranial vascular diseases.However,the catheter method for the injection of angiography agent during the operation of DSA is invasive for patients,which often leads to the occurrence of related complications.MRI examination time is long,postoperative evaluation is not good,and for the early detection of intracranial aneurysms is difficult,so it can not be used as a common method of census.CTA is a non-invasive vascular imaging technology,which has the advantages of low cost,convenient operation and low incidence of complications.It can quickly obtain the anatomical images of cerebral vessels.Through the generated three-dimensional images,we can observe the target vessels from multiple angles,and display and analyze the characteristics of the details of the pathological vascular anatomy images through the local magnification method.Moreover,CTA has high temporal and spatial resolution,and the scanning speed is fast in the detection process,which is easy for patients to accept.It can provide objective reference for the diagnosis and perioperative evaluation of aneurysms.This study is expected to make a comparative analysis of the consistencybetween CTA and DSA findings in the diagnosis and anatomic measurement of intracranial aneurysms,and to explore the clinical diagnostic value of CTA in internal intracranial aneurysms.ObjectiveTaking the results of DSA as the gold standard,the consistency of CTA in detecting intracranial aneurysms,measuring results,displaying the relationship between aneurysm neck and parent artery,and perforator vessels with DSA was analyzed,and the diagnostic value of CTA for internal intracranial aneurysms was discussed.MethodsA total of one hundred and sixteen patients with suspected intracranial aneurysms received and cured by Pingdingshan second people’s Hospital from January 2017 to June2019 were retrospectively analyzed.By comparing the results of CTA and DSA,the detection rate,shape(including contour,size,location,etc.)of intracranial aneurysms and the relationship between aneurysm body and surrounding tissues were analyzed.The coincidence degree and difference between CTA and DSA results were compared,so as to explore the diagnostic efficacy of CTA for intracranial aneurysms.ResultsThe data of 105 patients with intracranial aneurysms diagnosed by DSA were analyzed and the results are as follows:(1)There were 113 aneurysms detected by CTA,in compare with DSA(116 aneurysms were detected by DSA),the number of false negative and false positive were found as 5 and 2 respectively.Besides,in these false negative results,there was no tumor with the maximum diameter greater than 3 mm,the sensitivity of CTA was 95.7%.Furthermore,the results of CTA and DSA were statistically analyzed,fisher exact test was used to compare the detection rate of the two methods,while the corresponding p value was greater than 0.05,the difference was not statistically significant.(2)Moreover,it is found in this research that,when two methods(CTA and DSA)were used to measure the maximum diameter and neck of aneurysms which had been diagnosed,SPSS was further used to match the results,t test verification,There was a high consistency between the two methods in measuring the maximum diameter of the tumor;but in contrast to the biggest neck tumors measuring,in tumor neck on the measurement of CTA and DSA in the measured values(x + s,Mm)were observed as 4.06±1.18 mm and3.54±1.15 mm,respectively.After statistical analysis,the P values obtained were less than0.05,the difference was satistically significant which shows that the results obtained by examination were significantly different with different examination methods(the measured value of CTA was greater than DSA).(3)From the perspective of the definition of neck region of the tumor,this study explored and compared the results of different examination methods and found that the image information obtained from DSA examination for tumor of neck region were in degree of generally clear and clear,however,apart from the two cases mentioned above there are also 6 other fuzzy images obtained from the CTA inspection,Through the further rank sum set test,the results of satistical analysis are shown as fowllows: the Z value is 2.889,and the P value is 0.004,which is significantly less than 0.05,The results showed that DSA was better than CTA in showing the relationship between aneurysmal neck and parent artery relationship and its surrounding perforating vessels.Conclusions1.CTA has high sensitivity in the detection of intracranial aneurysms,and the detection rate and the measurement results of the maximum diameter of the aneurysm are consistent with those of DSA;2.DSA is better than CTA in displaying the relationship between tumor neck and parent artery and perforator vessels.DSA is still the most valuable method in the diagnosis of internal intracranial aneurysms at present. |
