BackgroundHypertensive disorders of pregnancy(HDP)is a common and important complication in obstetrics.It is a group of diseases coexisting with pregnancy and elevated blood pressure with an incidence rate of about 5%-12%.Including pregnancy hypertension,preeclampsia,eclampsia and chronic hypertension complicated with preeclampsia and pregnancy complicated with chronic hypertension.At present,the etiology and pathogenesis of hypertensive disorder complicating pregnancy are still unclear.Epidemiological investigations and experimental studies have put forward a variety of etiological hypotheses,such as immune theory,vascular endothelial damage theory,genetic theory,nutrition deficiency theory and insulin resistance theory.Apelin and adiponectin are both adipokines.current studies show that apelin and adiponectin are related to insulin resistance and vascular endothelial protection.Apelin is a novel bioactive peptide.Apelin-13 is a major subtype in human plasma and plays a major role in human cardiovascular system.Studies have found that it is involved in regulating homeostasis in vivo,including regulating cardiovascular system homeostasis,dilating blood vessels,lowering blood pressure,regulating immunity,hypothalamus-pituitary-adrenal axis regulation and metabolic homeostasis.Apelin-13 can stimulate the production and release of NO by activating endothelial nitric oxide synthase(e NOS)of vascular endothelial cells,protect vascular endothelium and play a role in vasodilation.Adiponectin has the functions of anti-inflammatory protection of vascular endothelium,regulation of cytokines,insulin sensitization,anti-diabetes and anti-atherosclerosis functions.Adiponectin exerts protective effect on vascular endothelial cells through NO-c GMP signaling pathway.ObjectiveIn this study,we studied the concentration and correlation of apelin-13 and adiponectin in peripheral blood serum of hypertensive disorder complicating pregnancy and normal pregnant women,and their effects on hypertensive disorder complicating pregnancy,and explored the correlation between them and hypertensive disorder complicating pregnancy.MethodsThis experimental study collected 56 pregnant women with hypertensive disorder complicating pregnancy(PIH group)who underwent regular prenatal examination and delivered in hospital in fengqiu county Maternal and Child Health Hospital from August2018 to March 2019(including 20 pregnant women with severe preeclampsia)and 50 normal pregnant women(control group)(there was no difference in age,gestational week and body mass index between the two groups(P > 0.1).With the consent of the medical ethics Committee,4ml of venous blood before delivery was drawn and put into a coagulation accelerator.after centrifuging the blood plasma,the blood plasma was stored in a-80 degree refrigerator.The concentration levels of apelin-13 and adiponectin were measured by enzyme-linked immunosorbent assay.SPSS22.0 analysis was used for data statistics.T test and t’ test were used for average number comparison between the two groups.Pearson linear correlation analysis was used for correlation analysis.The difference was statistically significant with P < 0.05.Results1.Compared with the general clinical data of the two groups of pregnant women,there was no significant difference in age,gestational age and body mass index between the two groups;The newborn weight of pregnant women with hypertensive disorder complicating pregnancy was lower than that of normal pregnant women,and the difference was statistically significant(P < 0.05).2.The concentration of Apelin-13 in pregnant women with hypertensive disorder complicating pregnancy was higher than that in normal pregnant women,and the concentration of adiponectin was lower than that in normal pregnant women.The difference was statistically significant(P < 0.05).3.The concentration of apelin-13 in serum of pregnant women with severe preeclampsia is higher than that of patients with non-severe preeclampsia,and the concentration of adiponectin is lower than that of patients with non-severe preeclampsia.The difference between the two is statistically significant(P < 0.05).4.Adiponectin in serum of hypertensive disorder complicating pregnancy is negatively correlated with proteinuria and blood pressure(P < 0.05)and has no correlation with neonatal weight(P > 0.05).5.Apelin-13 in serum of pregnant women with hypertensive disorder complicating pregnancy is positively correlated with proteinuria and blood pressure(P < 0.05),but not with neonatal weight(P > 0.05).6.Serum apelin-13 level of pregnant women with hypertensive disorder complicating pregnancy is negatively correlated with adiponectin(r=-0.507,P < 0.01).ConclusionApelin-13 is positively correlated with hypertensive disorder complicating pregnancy,while adiponectin is negatively correlated with hypertensive disorder complicating pregnancy.Apelin-13 and adiponectin are expected to be biomarkers of hypertensive disorder complicating pregnancy and have certain guiding significance for prevention and treatment of hypertensive disorder complicating pregnancy in clinical practice. |
