Objective To analyze the risk factors of urosepsis after transurethral ureteroscopic lithotripsy.Methods 216 cases of ureteral soft lithotripsy from April 2015 to August 2017 were analyzed by retrospective analysis.They were divided into experimental group and control group according to whether urosepsis occurred after the operation.The basic data of patients were collected,including occupation,gender,age,test results of laboratory related indicators,stone size,operation time,whether the operation is associated with side injuries,whether it is associated with diabetes,irrigation pressure,intraoperative reflux,gender,urine culture results,drainage conditions and pre-operation Situation,etc.Univariate analysis as a comparison between two groups,Statistical power was P<0.05.Patients who were found to have statistically significant differences through univariate analysis were included in the multivariate logistic regression analysis.To analyze the risk factors of ureteral soft lithotripsy complicated with urosepsis.Results 1.There was no significant difference in age,course of disease,body mass index and occupational distribution between the experimental group and the control group(P > 0.05).2.The results of laboratory measurement showed that there was no significant difference in hemoglobin,albumin,CT value and creatinine level between the two groups.(P>0.05).3.The single factor analysis showed that the stone size and operation time in the experimental group were significantly longer than those in the control group(P < 0.05).The experimental group was complicated with diabetes mellitus,the proportion of female,positive urine culture,side injury during operation,the irrigation pressure was too high,the intraoperative reflux was not smooth,the drainage was not smooth,the potential urinary tract infection,ureteral obstruction and the intrapelvic hypertension was significantly higher than that of the control group,and the difference was statistically significant.(P < 0.05).4.Logistic regression analysis showed that the risk factors of urosepsis after retrograde intrarenal surgery were diabetes mellitus,excessive washing pressure,intraoperative reflux,operation time,stone size,gender,urine culture result,drainage condition,potential urinary tract infection before operation,ureteral obstruction,operative side injury and intrarenal pressure.Conclusions There are many risk factors for urosepsis after retrograde intrarenal surgery,such as diabetes mellitus,intraoperative pressure,intraoperative reflux,operation time,stone size,gender,urine culture results,drainage and potential preoperative urinary tract infection,ureteral obstruction,operative side injury,intrapelvic hypertension,etc.these basic conditions and intraoperative operations can cause uremic pyemia.Clinicians should be alert to the risk factors of uremic sepsis,actively regulate and avoid them so as to reduce the risk of urosepsis.
