| Objective:To analyze the relationship between the bone metabolism index and bone mineral density level of type 2 diabetes mellitus and the TCM syndromes.Methods:120 in patients with type 2 diabetes were selected, general data and experimental indicators were collected, and the patients were divided into syndrome differentiation groups according to their clinical manifestations.Statistical methods were used to explore the correlation between TCM syndromes and general data.Results1.Distribution of TCM syndrome types:average age and distribution of disease course of type 2 diabetes in each syndrome group distribution deficiency of Yin and Yang syndrome>qi stagnation and blood stasis syndrome>syndrome of liver and kidney deficit.Women had more cases than men in all three syndrome groups.2.Distribution of clini cal indicators among TCM syndrome groups:mean distribution of bone formation indicator PINP:liver and kidney deficit syndrome>deficiency of Yin and Yang3.Mean distribution of bone mineral density of lumbar vertebrae among all TCM syndromes groups:deficiency of Yin and Yang>liver and kidney syndrome group>qi stagnation and blood stasis syndrome group4.Relationship between bone mass distribution among three group TCM syndrome:The distribution of cases in the normal bone mass and bone mass reduction group:liver and kidney deficit syndrome>qi stagnation and blood stasis syndrome>syndrome of deficiency of Yin and Yang;Among the patients diagnosed with osteoporosis:qi stagnation and blood stasis>deficiency of Yin and Yang>liver and kidney deficit.5.Correlation between lumbar bone density and various indexes in patients with type 2 diabetes:lumbar bone density was negatively correlated with age and disease course, and positively correlated with BMI.Correlation between bone metabolism indexes and various indexes in patients with type 2 diabetes mellitus:age was negatively correlated with PINP,N-MID and β-CTx,C-peptide level was negatively correlated with 25-OHVD level, and glycosylated hemoglobin level was negatively correlated with N-MID.Conclusion1.In patients with type 2 diabetes, liver and kidney loss is the main symptom in the initial stage, and qi stagnation and blood stasis and deficiency of Yin and Yang are the main symptoms in the middle and later stage.2.The lumbar vertebra density level of qi-stagnation and blood-stasis syndrome group was the lowest. In clinical practice,it should be noted that type 2 diabetic patients should be treated with qi-regulating and blood activating drugs to prevent osteoporosis.3.BMI plays a protective role in maintaining bone mass, while in patients with type 2 diabetes, bone density declines wi th age and progression of the disease, and the aging reduces the overall bone metabolism level. In addition,long-term high glucose status is not conducive to bone transformation, and insulin resistance status has an adverse effect on the active vitamin D in vivo. |
