Clinical Study On The Treatment Of Neer Ⅲ Proximal Humeral Fracture With ShengXueBuSui Decoction And Locking Plate

Objective:To explore the effect of locking plate in the treatment of Neer Ⅲ proximal humerus fracture,early postoperative treatment combined with Sheng Xue Bu Sui Decoction plus and minus adjuvant treatment on the time of fracture healing,the degree of pain and swelling and the recovery of shoulder joint function.Methods:60 cases of Neer type Ⅲ proximal humeral fracture were collected from the Affiliated Hospital of Shandong University of traditional Chinese medicine from April 2018 to October 2019 The collected cases were randomly divided into observation group(30 cases)and control group(30 cases).The observation group was treated with open reduction and plate internal fixation combined with shengxuebusui Decoction in the early period of internal treatment;the control group was treated with open reduction and plate internal fixation.Results:All the patients were followed up for 6-12 months(average 8 months).There was no wound nonunion or infection during the study.The p value of age,gender,cause of injury,left and right components of affected limb and previous medical history of the two groups were all greater than 0.05,which was comparable.1.Pain score: observation group,preoperative pain score: 8.83 ± 0.699,postoperative24 h pain score: 5.47 ± 0.681,postoperative 1 week pain score: 2.73 ± 0.640,postoperative4 weeks pain score: 1.17 ± 0.592;control group,preoperative pain score: 8.87 ± 0.730,postoperative 24 h pain score: 5.57 ± 0.626,postoperative 1 week pain score: 3.23 ± 0.626,postoperative 4 weeks pain score : 1.60 ± 0.498 points.There was no significant difference between the two groups(P > 0.05)after the statistical analysis of 24-hour pain score before and after the operation,which was comparable;the pain score of 1 week and 4 weeks after the operation in the two groups was significantly reduced,and the observation group was more advantageous,and the difference after the statistical analysis was statistically significant(P < 0.05).2.Clinical healing time of fracture: observation group: 11.80 ± 1.126 weeks;control group: 12.67 ± 1.093 weeks.The clinical healing time of fracture in the observation group was significantly shorter than that in the control group(P < 0.05).3.Neer shoulder function score: observation group: 87.57 ± 4.345 points,excellent:10 cases,good: 18 cases,medium: 2 cases,excellent and good rate: 93.3%;control group:83.60 ± 5.691 points;excellent: 7 cases,good: 18 cases,medium: 5 cases,excellent and good rate: 82.3%.The score of the observation group was higher than that of the control group,and the difference was statistically significant(P < 0.05);the excellent rate of the observation group was higher than that of the control group,but the difference was not statistically significant(P > 0.05).Conclusion:The treatment of Neer Ⅲ proximal humeral fracture with proximal humeral locking plate has a good effect.Postoperative administration of Sheng Xue Bu Sui decoction can shorten the clinical healing time of the fracture,effectively reduce the postoperative pain around the shoulder joint,and promote the recovery of shoulder joint function.
