Effect Of Shenfu Injection On Early Fluid Resuscitation In Septic Patients With Carotid Artery Blood Flow Corrected Time

Objective:To evaluate the effect of shenfu injection on early fluid resuscitation in patients with sepsis by carotid correction flow time.Methods:This study choose in January 2019-December 2019 at shandong university of Chinese medicine affiliated hospital intensive medicine and the east end of ICU patients with sepsis,the sifting eligible patients with 60 cases,by random indicator method is divided into the treatment group and control group,each group 30 cases and control group according to the guide for standard western medicine treatment,treatment group given standard treatment at the same time combined with western medicine and injection.Respectively to compare the treatment group and control group in HR,MAP,CVP,central venous oxygen saturation correction（Scv O₂）,carotid artery blood flow time（FTc）,lactic acid（Lac）,lactate clearance rate,vascular active drug dosage index before and after treatment,such as the change of 3 h,6 h,24 h,7 dsofa score before treatment and after treatment,symptom scores of traditional Chinese medicine,APACHE Ⅱ score,and 7 d and21 d case fatality rate.Results:In comparison between groups,MAP,CVP,Scv O₂and lactic acid clearance rate of the treatment group were all higher than that of the control group at 3h（MAP:P<0.05,CVP:P<0.05,Scv O₂:P<0.05,lactic acid clearance rate:P<0.01）.6h after treatment,the dosage of removing nail kidney in the treatment group was lower than that in the control group（P<0.05）,and the clearance rates of FTc,MAP,CVP,Scv O₂and lactic acid were higher than those in the control group（FTc:P<0.01,MAP:P<0.01,CVP:P<0.05,Scv O₂:P<0.01,lactic acid clearance rate:P<0.01）.After 24h of treatment,the dosage of HR,Lac,nail remover and kidney in the treatment group was lower than that in the control group（all P<0.05）,and the clearance rates of FTc,MAP,CVP,Scv O₂and lactic acid were higher than those in the control group（FTc:P<0.01,MAP:P<0.01,CVP:P<0.05,Scv O₂:P<0.01,lactic acid clearance rates:P<0.01）.At 7d,the scores of the treatment group were smaller than those of the control group（SOFA score:P<0.05,TCM symptom score:P<0.01,APACHE II score:P<0.05）.Conclusion:Shenfu injection can significantly improve the carotid FTc,microcirculation and tissue perfusion in patients with sepsis and septic shock,and has a better effect of fluid resuscitation,which has a protective effect on patients with sepsis.
