A Real World Study On The Treatment Of Children’s Hen?ch-sch?nlein Purpura With The Combination Of Traditional Chinese And Western Medicine

Objective:Based on the electronic medical record data of the Hospital Information System(HIS)of the Hospital affiliated to Shandong University of Traditional Chinese Medicine,the law of disease differentiation,syndrome differentiation and medication in the real clinical environment of allergic purpura was explored.Method:Collect the children diagnosed with allergic purpura as the first diagnosis in the pediatric ward of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 1,2016 to January 1,2020,and extract the basic information,diagnosis and syndrome differentiation of the children from the HIS medical record system.Symptoms and signs and medication information are true,rich in dimension,and complete in structure.Enter the EXCEL table,use descriptive statistics,cluster analysis,complex network analysis and other statistical methods to study and analyze the syndrome differentiation,selection,prescription and medication rules,evaluation To explore the therapeutic effect in real clinic,explore the core medicine combination of traditional Chinese medicine for allergic purpura.Result:1.General situation A total of 343 children with allergic purpura were included,74 in 2016,81 in 2017,82 in 2018,106 in 2019,the male to female ratio was 1.35:1,with a high incidence of school age.2.There were 127 patients with pathogenic infections.The common pathogens of infection were EB virus,Mycoplasma pneumoniae,Hemolytic streptococcus,Helicobacter pylori,Candida albicans,cytomegalovirus,and Coxsackie virus.There were 42 cases of allergic purpura nephritis,9 cases of gastrointestinal bleeding,and 27 cases of complications occurred in children,in order of myocardial damage,liver function damage,epididymitis.3.The selection of prescriptions for TCM syndrome differentiation has increased the application of Yinqiaosan Prescription,Dachaihu Tang Prescription,and Guizhi Fuling Pill Prescription compared to the previous two years.And effectively reduced the use of hormones in skin type + abdominal pain,skin type + joint symptoms(P<0.05);effectively improved the children’s discharge syndrome score(P<0.05).4.Core drugs Cangzhu,Achyranthes bidentata,Coix seed,Licorice,Scutellaria baicalensis,Forsythia,sagebrush,peony bark,cork,red peony,paeony,windproof.Conclusion:1.HSP has a high incidence at school age,and the common complication is myocardial damage.2.The evolution of TCM dialectical thinking and the addition of syndrome-type prescriptions have reduced the use of hormones in skin purpura with abdominal pain or joint symptoms,and effectively improved the children’s syndrome score.3.The law of traditional Chinese medicine reflects the treatment principle of "purging dampness,clearing heat,cooling blood and removing blood stasis" in allergic purpura.
