Objective To explore the application effect of “1+1+1”contract management mode in community hypertension management,and to provide methods and ideas for community hypertension management.Methods 452 patients with hypertension from January 2019 to December 2019 in Pujiang Town,Minhang District,Shanghai were selected.They were divided into control group(n = 154)and signing group(n = 298)according to different management modes.The control group was treated with conventional management mode(no contract),and the observation group was treated with “1+1+1”contract management mode on the basis of the control group.Both groups completed 6 months of management.The self-efficacy of the two groups was evaluated by GSES before and after 6 months of management;The blood pressure levels(diastolic pressure,systolic pressure)of the two groups were measured by blood pressure measuring instrument.Results(1)The self-efficacy scores at 1 month,2 months,3 months,and 6 months after management were lower in both groups than before management(P<0.05);the observation group was 1 month,2 months,The 3-month and 6-month self-efficacy scores were lower than those in the control group(P<0.05);(2)The levels of SBP(129.46±10.43vs169.49±14.39)and DBP(90.41±7.34vs98.43±8.51)in the 6 months after management were lower than those in the 6 months after management(P<0.05);the observation group The levels of SBP(129.46±10.43 vs 154.39±12.31)and DBP(90.41±7.34 vs 96.34±7.98)at 6 months after management were lower than those of the control group(P<0.05);(3)Mental fluctuation scores at 6 months after management were lower at both groups than before management(P<0.05);Fluctuations in SAS(42.16±5.29vs62.14±5.43)and SDS(39.61±4.46vs45.79±5.34)scores were lower than those in the control group(P<0.05);(4)The awareness rates at 6 months after management were higher in both groups than before management(P<0.05);hypertension-related knowledge(281(94.30)vs130(84.42))in the observation group was 6 months after management,Hypertension hazards(288(96.64)vs135(87.66)),community management methods(290(97.32)vs132(85.71)),regular review(286(95.97)vs129(83.77))and blood pressure monitoring awareness rate(294(98.66)vs134(87.01))were higher than the control group(P<0.05);(5)Lifestyle improvement rate,blood pressure control rate,regular medication rate in the observation group 6 months after management All were higher than the control group(P<0.05);(6)Management grou p,management form,management effect,service status in the observation group after “1+1+1” contract management mode management for 6 months Degree and satisfaction with blood pressure control were higher than those in the control group(P<0.05);(7)The incidence of stroke(0.67vs1.95),myocardial infarction(0.34vs1.30),heart failure(1.01vs1.95),chronic kidney disease(CKD)(2.01vs5.19),and blurred vision(2.01vs5.19)were lower in the observation group 6 months after management In the control group(P<0.05);(8)The quality of life scores at 6 months after nursing were higher in both groups than before care(P<0.05);PF(87.49±6.64 vs73.25±5.61),RP(85.21±6.43 vs77.84±6.13),BP(89.45±6.67vs78.45±6.54),GH(86.56±6.45vs78.41±6.75),VT(87.12±6.62vs75.49±6.51),The scores of SF(84.57±6.12 vs75.64±6.13),RE(86.74±6.51vs79.45±6.12)and MH(88.35±6.30 vs78.46±6.21)were higher than those of the control group(P <0.05).Conclusion “1+1+1”contract management mode used in community hypertension management can improve the self-efficacy of patients,reduce the level of blood pressure,improve the negative psychology of anxiety and depression of patients,improve the awareness rate of hypertension related knowledge,life style improvement rate,blood pressure control rate,regular medication rate and patient satisfaction rate,and reduce the incidence of hypertension related complications.It is helpful to improve the quality of life of patients and is worthy of popularization and application.
