| Objective:This study was mainly about the clinical characteristics of eosinophil infiltrating chronic sinusitis and the improvement of symptoms after functional endoscopy, and compared the clinical differences between ECRS and Non-eosinophil infiltrating chronic sinusitis. Methods:By analyzing the clinical data of patients with chronic sinusitis(Chronic Rhinosinusitis,CRS)and following up for 1 month,3 months,6 months and 1 year after fess,we analyzed the improvement of symptoms in 120 patients with chronic sinusitis from August 2016 to July 2017.The improvement of symptoms in 120 patients with chronic sinusitis from August 2016 to July 2017 was analyzed retrospectively.Eosinophilic granulocyte(Eosinophils,Eos)>27%in pathological paraffin section of nasal cavity after operation(47 cases of ECRS,73:Not-ECRS).:a report of 47 cases)The sinus lesions in ct films were evaluated by Lund-Mackay CT score of paranasal sinuses, the nasal symptoms after sinusitis were evaluated by vas score, and the systemic symptoms were evaluated by snot-20.Univariate ANOVA and chi-square test were used to compare the clinical features of the two groups before operation and the improvement of symptoms in different time after operation. Results:Compared with Non-ECRS group, the severity of symptoms in ECRS patients, the proportion of AR and asthma history were significantly higher, the incidence of bilateral nasal polyps was higher. The level of IgE in peripheral blood of ECRS patients was significantly higher than that of ECRS patients. The eosinophil infiltration in nasal polyps was significantly related to the degree of eosinophil infiltration, and could be used as a reference index for preoperative diagnosis of ECRS.In the CT score, the patients with ECRS were mainly treated with ethmoid, the patients with Non-ECRS were maxillary sinus and anterior ethmoid sinus, and the degree of OMC lesion was higher in Non-ECRS group. The local and systemic symptoms of ECRS patients improved significantly within one year after FESS.However, nasal congestion and snot symptoms in ECRS group increased 6 months after operation. Conclusion:There were differences between ECRS group and Non-ECRS group in preoperative symptoms, hypersensitivity status, degree of lesion involvement and improvement of postoperative symptoms. The specific classification criteria should be improved to guide clinical diagnosis and treatment. |
