ObjectivesTo compare and analyze the changes in treatment options and the prognostic factors in extranodal NK/T cell lymphoma(ENKCL)patients.Materials and Methods:This is a retrospective analysis and Ethics Council from Fujian Medical University Affiliated Union Hospital(Project NO.2020KY033)of the clinical data from 107 patients with ENKCL,diagnosed and treated from January 2007 to December 2016 in the Fujian Medical University Affiliated Union Hospital.Clinicopathological parameters,treatment,and 2-year survival rate were analyzed.The 107 patients included 79 males and 28 females,with a median age of 43 years(9~75 years)and an average follow-up time of 24 months(1~145 months).ResultsOverall survival(OS)in 2013~2016 is higher than before,respectively.The main chemotherapy regimens were: CHOP,SMILE,E-CHOP,PEG-ASP+Gemox+Thalidomide,etc.The selection rate of L-asparaginase or asparaginase-containing regimen also showed an increasing trend with time.The 1-and 3-years OS rate in patients receiving L-asparaginase was significantly higher than that of patients who were not treated with L-asparaginase(P ≤0.001).The 1-year and 3-year OS rate was higher in patients with no group B symptoms,no bone marrow infiltration,normal EBV-DNA viral load,and normal serum LDH levels than in patients with group B symptoms,bone marrow infiltration,elevated EBV-DNA viral load and elevated serum LDH levels.The differences were statistically significant for all criteria.ConclusionsIn recent years,the survival rate of patients with ENKCL diagnosed and treated in our hospital has been increasing with time,which coincided with a trend to greater use of chemotherapy regimens containing L-asparaginase.We speculate that this change in treatment options may have implications for improving patient survival.Bone marrow infiltration,group B symptoms,EBV-DNA viral load,and serum lactate dehydrogenase levels were associated with survival prognosis and were independent prognostic factors,suggesting they can be used as indicators for clinical diagnosis and treatment. |
