Objective:To investigate the effects of Botulinum toxin type A(BoNT-A)injection therapy comprehensive rehabilitation training on improving the muscle tone of patients with upper extremity spasm after stroke on patients’ motor function,ability of daily life,and emotional disorder,which can provide effective treatment for improving the quality of stroke patients life and reintegrate them into society.Methods:27 patients with upper limb spasm early after stroke were selected,which were divided into control group(16 cases)and treatment group(11 cases).Two groups of patients received comprehensive rehabilitation training,physical factor therapy,occupational therapy and other comprehensive rehabilitation training on the basis of conventional secondary preventive drug treatment.The treatment group received ultrasound-guided local injection of BoNT-A on the basis of the above rehabilitation training.The course of observation in this study was 12 weeks.Modified Ashworth Scale(MAS)and Wo1 f Motor Function Test Scale(WMFT),which was used to evaluate upper limb function before and after treatment at 1,4,8,and 12 weeks after treatment in both groups.Barthel Index Scale(BI),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS)to assess patients’ daily life abilities and changes in anxiety and depression.The above scale was independently evaluated before and after treatment by 2 professionally trained clinicians who did not know the grouping.SPSS19 software was used to analyze the data.Result:(1)Comparison of the MAS and WMFT scores before and after treatment in the two groups was P<0.05,that is,there was a statistical difference before and after treatment in the two groups.At week 4,8 and 12 after treatment,the MAS and WMFT score of the treatment group was compared with that of the control group,and the differences were statistically significant(P<0.05).(2)Comparison of BI score between the two groups before and after treatment showed statistically significant differences(P<0.05).The BI scores of the two groups at week 1,4,8 and 12 after treatment showed statistically significant differences(P<0.05).(3)SAS scores of the two groups were compared before and after treatment,and the differences were statistically significant(P<0.05).Comparison of SDS scores between the two groups before and after treatment showed no statistically significant difference between the control group(P>0.05)and the treatment group(P<0.05).SAS and SDS scores were compared between the two groups 4,8 and 12 weeks after treatment,and the differences were statistically significant(P<0.05).Conclusions:BoNT-A in the treatment combined rehabilitation training,which can improve its motor function and daily living ability,and reduce the incidence of emotional disorders after stroke,and improve the Quality of life(QOL)of patients,which is superior to the single treatment.
