On The Basis Of IR,the Clinical Research Of Yinchen Wuling Powder Plus Astragalus In The Treatment Of Spleen Deficiency,Phlegm-dampness And Obese Type 2 Diabetes Was Discussed

ObjectiveThis study using the Yin Chen Wuling powder plus astragalus to(spleen deficient)phlegmy wet obese type 2 diabetes patients for intervention treatment,observating the change of the serum methylglyoxal(MG)and other indicatorson before and after treatment,from the angle of insulin resistance,to discuss the possible mechanisms of Yin Chen Wuling powder to cure obese type 2 diabetes,so as to provide a scientific basis for clinical application of traditional Chinese medicine treatment of obese type 2 diabetes.MethodsA prospective randomized controlled clinical trial was conducted to collect a total of 66 patients who accorded with the inclusion and exclusion criteria in outpatient and inpatient treatment in endocrinology department of Foshan hospital of traditional Chinese medicine from March 2019 to December 2019,and divided into a control group of 31 and an experimental group of 35.Both groups were given health education,diet management and exercise therapy as routine basic treatment.Control group 31 cases:basic hypoglycemic program;Study group 35 cases:on the basis of the control group,using Yin Chen wuling powder plus astragalus,the course of treatment is 12 weeks.To observe and monitor the serum methylglyoxal,general observation index(gender,age,course of the disease,H height,W weight,body mass index(BMI),blood sugar related indicators(glycated hemoglobin HbAlC,FPG fasting glucose,fasting insulin Fins,Homa insulin resistance index-IR=(Fins(FPG)/22.5),Chinese medicine symptom score,the change of clinical curative effect evaluation indexes.Finally,the statistical analysis of all the data was conducted to explore the possibility and mechanism of yinchen wuling powder plus astragalus to treat obese type 2 diabetes from the perspective of insulin resistance,so as to promote the advantages of TCM in the treatment of obese type 2 diabetes patients and provide reliable clinical basis for TCM to reverse obese type 2 diabetes.Results1.There was no statistical significance between the two groups in terms of sex ratio,age,course of disease,H,W,BMI and other general data before the experiment(P>0.05).2.After treatment,Methylglyoxal MG decreased significantly in two groups(P<0.05),and MG decreased more significantly in the experimental group than the control group,with statistically significant difference(P<0.05).BMI in both groups significantly decreased after treatment(P<0.05),while in the experimental group was significantly low(P<0.05).HbAlC in both groups was significantly decreased after treatment(P<0.05),and HbAlC in the experimental group was significantly low(P<0.05).After treatment,FPG was significantly decreased in both groups(P<0.05),and FPG in the experimental group falling was more pronounced than in the control group(P<0.05).Both groups had significantly lower Fins after treatment(P<0.05),and the decline of Fins in the experimental group was more pronounced than in the control group(P<0.05).IR was significantly decreased in both groups after treatment(P<0.05),the decline of IR was more pronounced in the experimental group(P<0.05).The TCM syndrome score of the control group and the experimental group decreased significantly after the treatment(P<0.05),and the TCM syndrome score of the experimental group decreased significantly compared with the control group(P<0.05).3.Comparison of clinical efficacy,the total effective rate of the control group was 41.9%,while the experimental group was 74.3%.It’ s already statistically significant,because the clinical efficacy of the experimental group was better than the control group(P<0.05).the incidence of adverse reactions in both groups was lower,the control group was 9.7%,while the experimental group was 5.7%.ConclusionYinchen wuling powder and astragalus can effectively improve the insulin resistance and other related indicators of patients,and also reduce the content of methylglyoxal,an innovative indicator,suggesting that the latest treatment ideas of glycosphrosis can be explored from the perspective of cell metabolism.Study found that Yinchen wuling powder plus astragalus can fully improve phlegmy wet spleen deficient in patients with clinical symptoms of TCM,visible in the treatment of traditional Chinese medicine has a unique advantage in the obese type 2 diabetes,and less side effects and adverse reactions,and is worth popularizing by TCM syndrome after applied to clinical.Moreover,it can effectively identify the mechanism of action of methylglyoxal from the Chinese native medicine ingredients,and to promote the innovative development of traditional Chinese medicine and contributed to the modernization of traditional Chinese medicine.
