Research On Efficacy And Mechanism Of Compound Danshen Dripping Pills Treating Early Type 2 Diabetic Kidney Disease Via Gas6/Axl Pathway

ObjectiveThe study observes the clinical effect and safety of Compound Danshen Dripping pills on early type 2 diabetic kidney disease with blood stasis syndrome,discussing its functional mechanism via Gas6/Axl signal pathway.MethodsA total of 80 cases of early type 2 diabet.ic kidney disease with blood stasis syndrome were collected from patients admitted to the endocrinology department of Foshan hospital of traditional Chinese medicine from January 2019 to September 2019.The patients were randomly divided into control group and observation group,with 40 cases in each group.Both groups were given the routine basic treatment,including diabetes education,diet and exercise intervention,conventional hypoglycemic treatment,anti-hypertension therapy and lipid lowering therapy if necessary.Besides the basic treatment,the observation group were treated with Compound Danshen Dripping pills,10 grains Po three times a day with a course of treatment for 24 weeks.Changes of Urinary Albumin Creatinine Ratio(UACR),urine β 2-microglobulin(β2-MG),urine microalbumin(UmAlb),serum growth arrest-specific 6(Gas6),serum Axl Receptor Tyrosine Kinase(Ax1),serum Transforming Growth Factor-β1(TGF-β1),serum Methyglyoxal(MG),fasting blood glucose(FPG),glycated hemoglobin(HbAlc),blood lipid,and TCM syndrome score before and after treatment were observed.The data were analyzed to evaluate the effectiveness and safety of Compound Danshen Dripping pills.Results1.A remarkable difference was shown between the two groups in syndrome score(P<0.01)after treatment.The total effective rate was 71.8%in the control group,while 91.9%in the observation group.By Fisher chi square test,a significant difference was observed in the effective rate between the two groups(P<0.05).2.No obvious difference was shown in UACR,β2-MG,and UmAlb between the two groups before treatment,which was comparable(P>0.05).After treatment,there were remarkable differences in UACR,β2-MG and UmAlb between the two groups(P<0.01).There were significant differences in UACR,β2-MG,and UmAlb in both the control group and the observation group before and after treatment(P<0.05).3.No significant difference was shown in Gas6,Axl,TGF-β land MG between the two groups before treatment,which was comparable(P>0.05).After treatment,differences in Gas6,Ax1,TGF-β land MG between the two groups were pronounced(P<0.05).Differences in Gas6,Ax1,TGF-βland MG in both the control group and the observation group before and after treatment were remarkable(P<0.05).4.No significant difference was found in the levels of serum lipids(TC,TG,HDL-c,LDL-c)between the two groups before treatment,which was comparable(P>0.05).After treatment,differenc.es in TC,LDL-c between the two groups were obvious(P<0.01),while no visible difference was observed in TG,HDL-c between the two groups(P>0.05).Differences in TC,TG,LDL-c in both the control group and the observation group before and after treatment were pronounced(P<0.01),while no similar difference was found in HDL-c(P>0.05).5.No visible difference was demonstrated in FPG and HbAlc between the two groups before treatment.,which was comparable(P>0.05).Obvious difference was shown in HbAlc after treatment in observation group(P<0.01),while no significant difference was found in FPG between the two groups after treatment(P>0.05).Difference in HbAlc and FPG before and after treatment in observation group was pronounced(P<0.05),while no similar significant.difference was seen in control group(P>0.05).6.Safety analysis:no obvious abnormality was seen in the test of patients’blood,urine,stool,liver,kidney,and electrocardiogram;meanwhile,no patient had showed any drug allergy reactions such as fever,skin rash in the process of treatment.ConclusionCompound Danshen Dripping pills could relieve TCM syndromes and improve quality of life remarkably in patients suffering from early type 2 DKD with blood stasis syndrome.Obvious adverse reactions were not shown in patients with early type 2 DKD,demonstrating that.Compound Danshen Dripping pills are safe and reliable in terms of the clinical application.By reducing the UACR,β2-MG,and UmAlb to relieve renal lesion.Besides,a potential mechanism of Compound Danshen Dripping pills treating early type2 DKD is through down-regulating Gas6/Axl signaling pathways,reducing the activity of TGF-β1,decreasing serum MG level as well to postpone the process of glomerulosclerosis and renal fibrosis.
