ObjectiveThrough prospective randomized controlled study,this project aims to understand the sleep structure of patients with tumor related insomnia,explore the improvement degree of acupuncture on the clinical syndromes and insomnia symptoms of patients with tumor related insomnia of the syndrome differentiation of heart and spleen deficiency,so as to improve the quality of life of patients,enrich the TCM pathogenesis theory of cancer-related insomnia,and provide new ideas and prescriptions for clinical practice Law.MethodsTaking the patients diagnosed as malignant tumor in the outpatient department of traditional therapy department and the ward of Oncology Department of University Town Branch of Guangdong traditional Chinese medicine hospital as the research object.Strictly in accordance with the diagnostic criteria,inclusion criteria and exclusion criteria of the subject,60 cases were screened in this study.Random cards and random envelopes were prepared by using the method of random number table.Envelopes were selected according to the time sequence of patients’ visits.According to the numbers,they were divided into treatment group and control group,30 cases in each group.On the basis of taking Jiawei Guipi Decoction,the treatment group was treated with acupuncture.Acupoints were selected:Baihui,Sishencong,shenting,Yintang,Shenmen,Neiguan,Zusanli,Sanyinjiao,Shenmai and Zhaohai.After the patients had the sensation of getting Qi,they were kept for 20min,three times a week,one course in two weeks,a total of two courses.The control group was treated with modified Jiawei Guipi Decoction.Based on the observation table of case information collection,Pittsburgh sleep quality index(PSQI)and TCM syndrome of deficiency of heart and spleen were used as the observation indexes.The changes of treatment scores,clinical symptoms and sleep improvement of the two groups were recorded before and after treatment.SPSS17.0 statistical software was used to analyze the clinical efficacy and draw a conclusion.ResultsIn the treatment group,there were 16 males and 14 females.63.3%of them lived in urban areas and 36.7%in rural areas.In the control group,there were 14 males and 16 females,56.7%of whom lived in urban areas and 43.3%in rural areas.Before treatment,there was no significant difference between the two groups in sex,age,residence,KPS score,tumor type,clinical stage and other data(P<0.05),and the baseline was comparable.The subjects included in the study were mainly patients with advanced tumors,including 23 cases of lung cancer(38.3%),9 cases of colorectal cancer(15%),6 cases of nasopharyngeal carcinoma(10%),8 cases of liver cancer(13.3%),6 cases of gynecological tumors(10.1%),and 8 cases of other malignant tumors(13.3%).In the evaluation of the overall efficacy of insomnia,after 4 weeks,the effective rate of acupuncture treatment group was as high as 90%,while the total effective rate of control group was 63.33%.There was significant difference between the two groups(P<0.05).The improvement degree of the treatment group was better than that of the control group(P<0.05).The difference of KPS between the two groups before and after treatment was statistically significant(P<0.05),which indicated that the behavior of tumor patients could be improved after acupuncture.Before the treatment,there was no significant difference in TCM syndrome score and PSQI total score between the two groups(P>0.05),suggesting that the two groups had better comparability.After 4 weeks,the difference between the two groups was statistically significant(P<0.05).However,according to the data of the two scores,the difference was statistically significant(P<0.05),indicating that the effect of the treatment group was better than that of the control group.Before treatment,the TCM syndromes and PSQI of the two groups were compared(P>0.05).Comparison between the two groups:after 4 weeks of treatment,acupuncture in the treatment group was superior to the control group in improving insomnia,palpitation,fatigue,food intake,forgetfulness,abdominal distention,loose stools and face color(P<0.05),but there was no significant difference in the symptoms of multiple dreams before and after treatment(P>0.05).After 4 weeks of treatment,compared with the control group,acupuncture in the treatment group improved sleep quality,reduced the time to fall asleep,lengthen sleep time,improved efficiency,reduce sleep disorders and improved daytime dysfunction,the difference was statistically significant(P<0.05),suggesting that compared with the control group,acupuncture in the treatment group can improve clinical symptoms and sleep more comprehensively(P<0.05),and the effect is better.Analysis of sleeping drug use:compared with before treatment,the number of sleeping drugs used in the two groups was significantly reduced,the difference was statistically significant(P<0.05),suggesting that the treatment of the two groups can significantly reduce the dependence of tumor patients on such drugs.Safety evaluation:in the course of the study,60 tumor patients did not have adverse reactions,and the safety rating was 1.ConclusionThe effective rate of acupuncture in the treatment of cancer-related insomnia is high.Acupuncture can improve the overall sleep quality of patients,relieve insomnia,palpitation,mental fatigue,food intake,forgetfulness,abdominal distension,loose stools,complexion and other TCM symptoms of heart and spleen deficiency.Acupuncture is safe and free of adverse reactions.Combined with traditional Chinese medicine,it can improve the effect,reduce the use of Western medicine sleeping pills,improve the behavior of patients with malignant tumors,and improve the quality of life.
