| ObjectiveCombined with the STAF score scale,the characteristics of thepopulation of patients with transcranial Doppler ultrasound bubble test and to explore the type of yin and yang.To explore the scope of application of transcranial Doppler ultrasound bubble test and the possible pathogenesis of cryptogenic stroke provide a reference for clinical etiology screening and the formulation of secondary prevention programs.Methods1.This study selected patients with acute ischemic stroke who completed the TCD foam test from November 2017 to January 2020 in the University City Hospital of Guangdong Provincial Hospital of Traditional Chinese Medicine.Cryptogenic stroke patients who meet the inclusion criteria are divided into 2 groups according to the TCD foaming test results:TCD foaming test(+)group and TCD(-)group;general clinical data of the two groups of patients are collected:cerebrovascular Disease risk factors(age,gender,hypertension,diabetes,hyperlipidemia,smoking history,drinking history),infarct distribution characteristics and STAF scores were compared between groups to complete statistical analysis and draw conclusions.2.Analyze whether there is statistical significance in the classification of TCM yin and yang syndrome in two groups.Results1.The general results of this data show that there are a total of 64 patients with cryptogenic stroke,including 30 cases in the positive group and 34 cases in the negative group.The positive detection rate of cTCD is 46.9%;there are 45 males,accounting for 70.3%,and 19 females.Accounted for 29.6%,cryptogenic stroke was dominated by men;the general clinical data and cerebrovascular risk factor statistics of the two groups of patients were compared:gender(positive:male:19 cases,accounting for 29.7%,negative:26,accounting for 40.6%);Hypertension(40 cases,62.50%,positive:16 cases,25.0%,negative:37.5%);diabetes(12 cases,18.7%,positive:4.7%,negative 14.1%);hyperlipidemia(18 cases,accounting for 28.1%,positive:9.4%,negative:18.8%);smoking(22 cases,accounting for 34.4%,positive:14.1%,negative:20.3%);drinking(6 cases,accounting for 9.4%,positive:0 Cases,0.0%,negative:6 cases,9.4%);high homocysteine(9 cases,14.1%,positive:6.3%,negative:7.8%);the above data shows that common risk factors for cerebrovascular disease The proportion of cryptogenic stroke is relatively low,except for hypertension Its proportion is 62.5%;comparing the ratio of risk factors of cerebrovascular disease in Yin and Yang groups,the proportion of positive group is lower than that of negative group.However,the results of statistical analysis showed that there was no significant difference in gender,age,hypertension,diabetes,hyperlipidemia,smoking history,and homohomocysteine,P>0.05,which was not statistically significant.Statistical significance,P<0.05;NIHSS score and infarction distribution characteristics of the two groups were not significantly different,P>0.05,no statistical significance.2.The statistical analysis of STAF in the two groups showed that there was a significant difference in the STAF score scale between the two groups.The mean ± standard deviation of the STAF score in the positive group was 2.60±1.75,and the mean ± standard deviation of the STAF score in the negative group was 1.76±1.74;The STAF score of the positive group was higher than that of the negative group.Statistical analysis of the two groups showed that P=0.042<0.05,which was statistically different.3.Statistical analysis of STAF was performed in the two groups,and the results showed that there was a significant difference in the STAF score scale between the two groups,and the score of the STAF scale in the positive group was higher than that in the STAF negative group,P=0.042<0.05,which was statistically different.4.Statistical analysis of yin and yang types was performed in two groups of patients,and the results showed no statistical significance between the two groups of yin and yang types.Conclusion1.The results of this study suggest that the population characteristics applicable to cTCD are cryptogenic stroke patients with relatively few cerebrovascular disease risk factors and relatively high STAF scores.Combining the above conclusions can indirectly confirm that the pathogenesis of cryptogenic stroke may indeed be related to PFO-induced arrhythmia;2.For patients with low cerebrovascular risk factors and high STAF scores,cryptogenic stroke populations need to undergo TCD foaming test.If conditions permit,long-term ECG monitoring and further improvement of transesophageal echocardiography are feasible.Anti-coagulation therapy is the first choice;3.There is no statistical significance between the two types of Yin-Yang syndrome types,which reflects the difference between the overall concept of TCM differentiation and the Western medicine microdiagnosis and treatment plan,so there is no obvious difference between the two groups of Yin-Yang syndrome. |
