Objective To investigate the clinical value of TI-RADS in the diagnosis of thyroid microcarcinoma.Methods From January 2018 to May 2019,a total of 296 patients diagnosed as TI-RADS grade 3-5 who underwent thyroid ultrasound examination in the ultrasound clinic of our hospital were selected as the subjects of this study.The clinical value of ultrasound TI-RADS grading in the diagnosis of thyroid microcarcinoma was studied through the retrospective analysis of the results of ultrasound TI-RADS grading,pathological examination and ultrasonic indications.Results 296 cases of small thyroid lesions were confirmed by pathological examination,88 cases(102 lesions)were malignant thyroid lesions,181 cases(194 lesions)were benign thyroid lesions.Among the malignant patients,63 cases were younger than 45 years old,accounting for 71.6% and 25 cases were older than 45 years old,accounting for 28.4%.The sensitivity,specificity,accuracy,negative predictive value and positive predictive value of ti-rads in the diagnosis of thyroid microcarcinoma were 86.3%(88 / 102),83.0%(161 / 194),84.1%(249 / 296),92.0%(161 / 175)and 72.7%(88 / 121)respectively.In this case study,there were 7 ultrasound indications,including rough margin,irregular shape,microcalcification in the focus,aspect ratio ≥ 1,low echo in the focus,cervical lymph node enlargement and rich blood flow signal in the focus.There were significant differences among the 7 ultrasound indications(P < 0.01).The accuracy of 7 ultrasound indications in the diagnosis of thyroid microcarcinoma ranged from 73.6% to 80.1%.Among the indications,microcalcification,rich blood flow signal,aspect ratio > 1 and cervical lymphadenopathy had high specificity,which were 90.7%,92.3%,89.7% and94.3% respectively.The sensitivity of rough margin,irregular shape and solid hypoecho in the focus were 79.4%,77.5% and 94.1%,respectively.Conclusion Ultrasound TI-RADS grading can accurately evaluate the benign and malignant of thyroid micro lesions,and it has significant clinical value in the early diagnosis and treatment of thyroid micro cancer,which is worth popularizing and applying.
