Study On Traditional Chinese Medicine Constitution Of Obese Patients With Diabetic Lower Extremity Arterial Disease And Its Relation With Inflammatory Indicators

Objectives To analyze the TCM constitution types of diabetic lower extremity arterial diseases and explore the correlation between TCM constitution and inflammatory indexes in obese patients.Methods 240 diabetic patients with lower extremity arterial disease were selected from January 2017 to December 2018 in the Department of Peripheral Vascular Diseases,Affiliated Hospital of Shandong University of TCM.All patients were grouped according to body mass index(BMI),and record the general information of patients and laboratory indicators,and judge the constitution of TCM by classification of constitution of TCM.SPSS20.0 was used to carry out Kruskal-Wallis H test,X2-test and logistic regression analysis.Results 240 diabetic patients with lower extremity arterial disease were enrolled,including 159 males and 81 females,with an average age of 66.33years(41-86 years).Among the 9 types of constitution,there were 32 cases of Peaceful constitution and 208 cases of biased constitution,among which Yin deficiency constitution(55.00%),damp-heat constitution(46.67%),phlegmdampness constitution(41.25%)and blood stasis constitution(39.17%)were the most common.X2-test of constitution and other factors showed that the distribution of constitution was significantly different from gender,body mass index,smoking,drinking,hypertension and cardiovascular disease and blood lipid indexes(P<0.05).Among 80 obese patients,49 were males and 31 were females.The constitutional types of TCM included Yin deficiency(23.66%),damp-heat constitution(23.22%)and phlegm-dampness constitution(19.62%).The correlation between constitution and inflammatory indexes was analyzed.Damp-heat quality was positively correlated with C-reactive protein,while damp-heat quality,blood stasis quality and plasma fibrinogen were positively correlated;Logistic regression analysis showed that the risk of obesity in phlegm-dampness constitution and damp-heat constitution was significantly higher than that in peaceful constitution.Conclusions Diabetic lower extremity arterial disease is mainly accompanied by biased constitution,the constitution type is mainly Yin deficiency constitution and damp-heat constitution;Phlegm-damp constitution and damp-heat constitution easily DLEAD to obese diabetic lower extremity vascular disease,Damp-heat is easy to occur when the level of C-reactive protein is abnormally increased,while damp-heat and blood stasis are easy to occur when the level of plasma fibrinogen is abnormally increased.
