Objective:To analyze the correlation between coagulation,fibrinolysis and urinary protein in children with glomerular disease,and to analyze the influence of related factors in the progression of renal disease in children.method:Eighty-six patients with kidney disease who were admitted to the Department of Pediatrics,First Hospital of Shanxi Medical University from 2016 to 2018,including Nephrotic Syndrome(NS),IgA nephropathy,Purpura Nephritis,and chain were selected.After the infection of glomerulonephritis(APSGN),non-Poststreptococcal Glomerulone phritis,the basic conditions of the patients include age,gender,and uniform instruments after morning blood draw The biochemical indicators,coagulation and fibrinolysis indicators were detected,the patient’s urine was collected for 24 hours,and the 24-hour urine protein was measured.According to the 24-hour urine protein content,it was divided into 5 groups: 20 cases of urinary protein negative group,10 cases of urinary protein positive group 1,9 cases of urinary protein positive group 2,23 cases of urinary protein positive group 3,and 24 cases of urinary protein positive group 4 The variance analysis and rank sum test were used to compare the clinical data of each group and the differences of coagulation and fibrinolysis indexes.The correlation between 24-hour urine protein content and coagulation and fibrinolysis indexes was further analyzed by linear correlation and Spearman correlation analysis.Statistical analysis was performed on the obtained data using SPSS 21.0 statistical software.result:(1)84 subjects were studied,including 47 cases of NS,8 cases of IgA nephropathy,13 cases of purpuric nephritis,14 cases of APSGN,and 2 cases of glomerulonephritis after non-streptococcal infection.Divided into 5 groups: urine protein negative group 20 cases,male 11 cases,female 9 cases;urine protein positive group 1 10 cases,male 3 cases,female 7 cases;urine protein positive 2 groups 9 cases,male 7 cases,female 2 There were 23 cases of urinary protein positive in 23 groups,17 males and 6 females;22 of 4 patients with urinary protein positive,16 males and 6females.(2)urine protein negative,urine protein positive group 1,urine protein positive group 2,urine protein positive group 3,urine protein positive group 4 group compared with group ALB,TC,TG,α1-MG,BUN,Ig G,ESR The difference was statistically significant(P<0.05).There was no significant difference in age,β2-MG,Scr and SUA between the 5 groups(P>0.05).(3)NS,IgA nephropathy,purpuric nephritis,other nephritis(including APSGN,glomerulonephritis after non-streptococcal infection)showed a statistically significant difference in the abnormal rates of PT,AT-III,and Fib(P< 0.05).(4)Urine protein levels were independent variables.Linear correlation and Spearman correlation analysis showed that urine protein was negatively correlated with ALB,Ig G,PT and AT-III(P<0.05);and TC,TG,α1-microspheres.Protein,ESR and Fib were positively correlated(P<0.05),and the difference was statistically significant.conclusion:1.The abnormal rates of PT,AT-III and Fib in coagulation and fibrinolysis indicators of various types of glomerular diseases are different,but the probability of abnormalities in APTT,TT and D-D is about the same;2.The glomerular disease proteinuria affects the coagulation and fibrinolysis status of children.As the degree of urinary protein increases,the coagulation tends to increase,which leads to imbalance of coagulation and fibrinolysis.Urine protein is negatively correlated with PT and AT-III.Fib is positively correlated. |
