A Comparative Study Of Internal Fixation And Artificial Hip Joint Replacement In Repair Of Intertrochanteric Fractures

Objective To investigate the effect on femoral intertrochanteric fracture among artificial hip replacement,proximal femoral nail antirotation and locking compression plate.Methods A retrospective study of 85 cases with unstable intertrochanteric hip fractures of aged patients.25 cases were treated with hip replacement,39 cases with proximal femoral nail antirotation and 21 cases with proximal femoral locking compression plate.The time of operative time,operative blood loss,visual analogue scale,average time of off-bed,hip function scores(Harris scores standard)were recorded and complication.Results Among the three groups,the artificial hip replacement group mean operative time of was(60.80±17.91)minutes,the mean operative blood loss was(173.3±39.2)milliliter,the mean visual analogue scale after a week was(3.51±0.54)scores,the average time of off-bed was(15.94±8.87)days,the mean hip function scores was(87.8±12.1)scores.In proximal femoral nail antirotation group,the mean operative time of was(56.50±23.30)minutes,the mean operative blood loss was(120.8±30.5)milliliter,The mean visual analogue scale after a week was(2.33±0.13)score,the average time of off-bed was(40.03±10.69)days,the mean hip function scores was(91.8±8.6)scores.In locking compression plate,the mean operative time of was(96.50±21.71)minutes,the mean operative blood loss was(220.4±40.6)milliliter,The mean visual analogue scale after a week was(2.59±0.27)score,the average time of off-bed was(60.24±11.18)days,the mean hip function scores was(90.22±7.9)scores.There were significant differences in operative blood loss,visual analogue scale and average time of off-bed in proximal femoral nail antirotation,locking compression plate and hip replacement for femoral intertrochanteric fracture(P<0.05).Compared with other groups,artificial femoral head replacement group of patients have relatively increased bleeding amount,lower VAS score after 1 week,the time in bed significantly shortened,the difference was statistically significant(P<0.05);Postoperative observation group 1 year Harris hip score and the incidence of postoperative total complications are significantly lower(P<0.05).Conclusion Compared with internal fixation group,Artificial hip replacement for complex femoral intertrochanteric fractures can effectively restore the joint function and help the patients leave the sickbed early,with few complications caused by a long-term bed rest.It is worth clinical promotion.
