Ultrasound-guided Percutaneous Transluminal Angioplasty In The Treatment Of Arteriovenous Graft Stenosis In Hemodialysis Patients:a Single-center Study On The Patency Rate Of 36 Patients

Objective To investigate the effectiveness and feasibility of ultrasound-guided percutaneous transluminal angioplasty(PTA)in the treatment of arteriovenous graft(AVG)stenosis.And to analyse influencing factors of patency rate after PTA.Methods Thirty-six patients with arteriovenous graft stenosis at the Third Affiliated Hospital of Bengbu Medical College from April 2018 to October 2019 were enrolled.By comparing the diameter of stenosis,AVG blood flow and brachial artery blood flow before and after the treatment of PTA to evaluate therapeutic effect.The patients were followed up for 12 months.The differences between primary patency rate and secondary patency rate at 3,6,9 and 12 months after PTA treatment were compared,and the possible factors affecting patency rate after PTA treatment were explored.Results1.Before the treatment of PTA,the diameter of stenosis was(1.58±0.35)mm,AVG blood flow was(144.13±19.12)m L/min and brachial artery blood flow was(352.30±42.01)m L/min,after then,the results were as follows:(3.71±0.62)mm,(275.75±14.29)m L/min and(616.83±45.92)m L/min,There were statistically significant differences between the three indexes before and after operation.(t=-16.61、-35.76、-28.26,P<0.01).2.The primary patency rate of 3,6,9,12 months was 80.6%,66.7%,47.2%,38.9%respectively,and secondary patency rate was 88.9%,75.0%,69.4%,58.3%respectively.There was no statistical significant difference in the primary patency rate and secondary patency rate among the follow-up times.(P>0.05).3.The age of patients in the unobstructed group was lower than that in the non-unobstructed group at 1 year postoperatively,and the difference was statistically significant.(P<0.01).There were no statistical significant differences in primary diseases such as chronic glomerulonephritis,hypertension,diabetes mellitus,gender,dialysis time and venous anastomosis between the unobstructed group and non-unobstructed group.(P>0.05).Conclusion Ultrasound-guided percutaneous transluminal angioplasty is effective to treat arteriovenous graft stenosis,when the age is older,the patency rate is lower.
