Purpose:Osteosarcoma is the most common primary malignant tumor of the bone,which mainly occurs in children and adolescents.The extremities are the most common sites of bone tumors,often in the distal femur and proximal tibia,mainly through blood metastasis.The main method for the treatment of osteosarcoma is surgical treatment,but single surgical treatment can not improve the survival time of patients,and the survival rate of patients with metastatic or recurrent osteosarcoma is still very low.The multidisciplinary model of surgery combined with radiotherapy and chemotherapy can effectively improve the prognosis of some patients,and in order to intervene patients with different risk levels and improve their prognosis,it is particularly important to better understand the prognostic factors of osteosarcoma of the extremities.The purpose of this study is to retrospectively analyze the clinical characteristics of postoperative osteosarcoma patients,and an accurate line chart is developed by selecting effective factors by single factor and multiple factors,and the survival time of postoperative osteosarcoma patients is predicted.This tool can provide effective information for doctors and patients.Methods:The relevant clinical variables of 805 postoperative patients with osteosarcoma diagnosed pathologically from 2004 to 2014 are collected through the Surveillance,Epidemiology and End Results(SEER)Database of National Cancer Institute.Major factors are age,sex,race,tumor location,marital status,tissue type,tumor grade,T stage,N stage,tumor size,mode of operation,chemotherapy,radiotherapy,lymph node resection,bone metastasis,lung metastasis and important follow-up materials(survival outcome and total survival time).The factors related to survival are obtained by univariate and multivariate COX analysis.To accurately predict the3-year and 5-year survival rate of postoperative patients with osteosarcoma.At the same time,the model is verified by internal data,and the accuracy of the model is further evaluated by C index and calibration chart.Results:In the survival analysis of the main treatment methods,whether chemotherapy or not has no significant effect on overall survival and tumor-specific survival(P>0.05);surgical methods,radiotherapy or not,and lymph node surgery have significant statistics on survival Significance(all P <0.05);single factor and multivariate analysis screened out age,tumor grade,T stage,N stage,tumor size,tissue type,lung metastasis,tumor primary site and radiotherapy are independent factors that affect overall survival Risk factors.Based on these significantly related factors,a nomogram model of 3-year and 5-year survival rates is established.The C index of the modeling group and the verification group are both greater than 0.7.In addition,the calibration charts also show the survival of the modeling group and the verification group.The consistency of the rates is good.Conclusions:In this study,we established an accurate model of patients and survival rate after osteosarcoma surgery based on osteosarcoma patients in the SEER database,which is of great significance for clinicians to formulate further treatment plans.
