ObjectiveTo explore the risk factors of recurrence of primary trigeminal neuralgia after microvascular decompression.MethodsAccording to the purpose of the study,according to selection criteria and exclusion criteria,408 patients with primary trigeminal neuralgia and microvas-cular decompression for trigeminal neuralgia diagnosed in our hospital from October 2010 to September 2018 were selected.According to the follow-up results,patients with trigeminal neuralgia after microvascular decompression were divided into two groups: non-recurrence group and recurrence group.Among them,356 cases in non-recurrence group and 52 cases in recurrence group.A retrospective case-control study was conducted.Relevant research information was collected from case records,treatment,preoperative pain survey,relevant preoperative examinations,surgical records,surgical video,post-operative effect evaluation,post-operative complications and post-operative follow-up.According to the clinical data and research purposes,the patients’ gender,age,side,branch,time of hypertension,course and type were screened and analyzed.11 factors including vascular type,degree of compression,number of vessels and degree of remission after operation were divided into two categories.Data were analyzed by SPSS17.0 statistical software.Firstly,the correlation between single factor and prognosis was analyzed by 2 test.The difference was statistically significant with P<0.05,and the factors that had obvious correlation with prognosis were screened out.Before introducing logistic multivariate analysis,in order to ensure the adequacy of samples collected and the accuracy of parameter estimation,EPV method,which is widely used at present,is applied to carry out sample size analysis to prove the adequacy of samples in this study.The factors related to prognosis are then introduced into binary logistic regression equation to carry out multivariate analysis.The difference is p<0.05,and the pairs are screened out.Independent risk factors that have a significant impact on prognosis,in the course of the study,in order to ensure the correctness of the data,the data were processed from many aspects.ResultsThe relationship between 11 factors and prognosis was analyzed by univariate analysis.The results showed that gender,age,side,branch,time of hypertension and number of blood vessels were not related to prognosis(p>0.05),but the course,type,type of blood vessels,degree of compression and degree of remission after operation were related to prognosis(p<0.05).Multivariate analysis showed independent risk factors affecting prognosis.Logistic regression analysis showed that the degree of compression was not related to prognosis(p>0.05).The course of disease,type of blood vessel,degree of remission after operation were related to prognosis(p<0.05),and significantly related to prognosis.ConclusionPatients with trigeminal neuralgia whose course of disease is more than 8 years,manifestations of atypical trigeminal neuralgia,responsible or irresponsible vessels,and no immediate relief of facial pain after operation are independent risk factors for recurrence. |
