ObjectiveComparative analysis of the safety and efficacy of simple balloon dilation or stent placement in patients with atherosclerotic tandem occlusion of the initial segment of internal carotid artery in acute stage.To explore the factors affecting the90-day prognosis of patients with acute anterior circulation tandem occlusion(atherosclerotic lesions of the initial segment of internal carotid artery combined with middle cerebral artery occlusion)cerebral infarction patients with endovascular treatment.MethodsIt was retrospectively included in the period from January 2017 to January 2020.He was admitted to the Department of Neurology of the First Affiliated Hospital of Hainan Medical College.Acute anterior circulation tandem occlusion(atherosclerotic initial segment of internal carotid artery)underwent endovascular intervention within8 hours of onset.The clinical data of patients with sclerosing lesions and middle cerebral artery occlusion)cerebral infarction are divided into stent group and balloon group according to the treatment of the initial segment of internal carotid artery in the acute phase.Analysis and comparison of the initial segment of internal carotid artery disease in acute phase are simple The safety and efficacy of balloon expansion and stent implantation are different.According to the 90-day mRS score,the patients were divided into a good prognosis group(mRS score 0-2)and a non-good prognosis group(mRS score 3-6).The factors affecting the 90-day clinical prognosis of patients in the above two groups were analyzed.Results1.There was no statistically significant difference in the 90-day clinical prognosis rate,symptomatic intracranial hemorrhage,asymptomatic intracranial hemorrhage,vascular restenosis,and mortality between the balloon group and the stent group(P>0.05).2.There was a statistically significant difference between the good prognosis group and the non-good prognosis group in age,preoperative ASPECT score,NIHSS score at admission,and early neurological improvement(P<0.05).Conclusions1.This study shows that for patients with acute anterior circulation tandem occlusion caused by atherosclerotic lesions in the initial segment of the internal carotid artery,the effectiveness and safety of simple balloon dilation or stent implantation in the acute phase are similar.2.This study shows that in patients with acute anterior circulation atherosclerotic tandem lesions treated by endovascular treatment,the elderly,low preoperative ASPECT score,high NIHSS score at admission,and early deterioration of neurological function are the influencing factors of their 90-day poor prognosis.
