Effect Of Medical Service Price Adjustment On Famliy Disease Economic Burden Of Uremic Children

In 2019,there are more than 3 million uremic patients in China,of which the proportion of children is increasing year by year.The economic burden of uremic children is relatively heavy.Since the launch of the new medical reform in 2009,the state has continued to promote the reform of medical service prices,including the adjustment of medical service prices in Chongqing for many times.Does the adjustment of medical service prices have an impact on the economic burden of family diseases of uremic children?Relying on the third class children’s Hospital and using the hospital information system,this paper investigates the children with ICD-10 disease diagnosed as"stage 5chronic kidney disease"（hereinafter referred to as uremia）.By comparing various medical expenses of uremic children from 2019 to 2021,this paper studies the impact of medical service price adjustment on medical expenses of uremic children;Five hospitalized children with uremia,five main caregivers and two medical staff were selected for case interviews;Combined with the demand theory of health and medical and health services and the theory of family disease economic burden,the direct medical cost,direct non-medical cost and indirect economic burden of disease of 28uremic children,as well as the frequency,intensity and average overspending level of family health catastrophic expenditure were calculated through questionnaire survey;The quality of life and family burden of disease of 28 uremic children were evaluated by child quality of life scale(Peds QLTM^4.0)and family burden of disease scale（FBSD）;The influencing factors of family catastrophic health expenditure,children’s quality of life and family burden of disease were analyzed.The study found that the medical burden of uremic children did not increase after the adjustment of medical service price in Chongqing,and the change of medical hospitalization cost structure of uremic children was basically consistent with the policy goal of medical service price adjustment,but it still needs to be further optimized;The economic burden of family diseases of uremic children is still very heavy;The incidence of family catastrophic health expenditure of uremic children is high,which is related to the length of hospital stay;The quality of life of uremic children and the burden of disease families are affected.According to the research results,it is proposed that the price adjustment of medical services should pay attention to the disease economic burden of vulnerable people;Chronic disease prevention and control measures for uremic children;And the assistance policies adopted by the potential"poverty caused by illness"groups;Suggestions on reducing the psychological burden of uremic children and families.
