Objective: Recent studies have have shown that NAFLD is related to the high prevalence of gastrointestinal polyps,but there are few studies on the relationship between the severity of NAFLD and gastrointestinal polyps.The purpose of this study was to explore the relationship between the severity of NAFLD and the characteristics of gastrointestinal polyps.Methods: The clinical data of patients with NAFLD diagnosed in the first affiliated Hospital of Xinjiang Medical University and improved digestive endoscopy were retrospectively analyzed.Patients with polyps were treated as NAFLD with polyps group(n=330)and patients without polyps as simple NAFLD group(n=210).The general data,biochemical indexes and imaging findings of the two groups were compared and analyzed.At the same time,the characteristics and biochemical indexes of intestinal polyps detected by different degrees of NAFLD in polyp group were compared.Logistic regression analysis was used to explore the correlation..Results:(1)The proportion of male,hypertension and diabetes in the polyp group was significantly higher than that in the non-polyp group,the average age of the polyp group was higher than that of the non-polyp group,and the BMI of the polyp group was higher than that of the non-polyp group.The BMI of the polyp group was higher than that of the non-polyp group.The glycosylated hemoglobin,fasting blood glucose,high density lipoprotein,indirect bilirubin,albumin,alanine aminotransferase,red blood cell count,hemoglobin,platelet count and D-dimer in NAFLD with polyp group were higher than those in NAFLD group.Age and BMI,ALB,D-Dimers are the risk factors of NAFLD complicated with gastrointestinal polyps.(2)There were significant differences in the proportion,average age and BMI of diabetes among mild,moderate and severe groups.The proportion of severe,moderate and mild gastric polyps was 44.29%,32.95% and18.6% respectively,and the proportion of intestinal polyps was 100%,78.4% and 87.21%,respectively.There were significant differences in TG、HDL、LDL、TB、IBIL、WBC、RBC、LY、Hb、PT among the three groups.(3)There was no significant difference in the location distribution of intestinal polyps and gastric polyps among mild,moderate and severe groups,but there were significant differences in the number,size and pathological types of intestinal polyps and gastric polyps among the three groups.The proportion of intestinal adenoma,proliferative intestinal polyp and inflammatory intestinal polyp in severe fatty liver group was 54.29%,27.14% and 18.57%,respectively,while the proportion of the above three types of polyps in mild and moderate fatty liver group increased in turn.The proportion of adenomatous gastric polyps(51.61%)in severe fatty liver group was higher than that of other pathological types.There was no significant difference in APRI score,FIB-4 score and NAFLD liver fibrosis score between different types of intestinal polyps and gastric polyps.(4)Logistic regression analysis showed that the risk of intestinal adenoma in patients with moderate and severe fatty liver was significantly higher than that in patients with mild fatty liver.the risk of intestinal adenoma in patients with severe fatty liver was 7.323 times higher than that in mild fatty liver.The risk of gastric adenoma was significantly higher in patients with moderate and severe fatty liver,and their OR values were 8.5 and 25.185 respectively.The difference was statistically significant.Conclusion:(1)NAFLD patients with gastrointestinal polyps were more likely to have hypertension and diabetes,were older,had a higher body mass index,and were more likely to be male.(2)Age,body mass index,albumin and D-dimer are risk factors for NAFLD complicated with gastrointestinal polyps.(3)NAFLD increased the prevalence of gastrointestinal polyps,and the prevalence of intestinal adenoma and gastric adenoma in patients with severe fatty liver was higher than that in mild and moderate fatty liver group.
