| Objective: to observe the effect of Buyang Huanwu decoction on the rehabilitation of cervical Spondylotic myelopathy after anterior cervical fusion(Anterior Cervical Discectomy and Fusion,ACDF),and to explore the effectiveness and safety of its application,so as to provide some ideas for the participation of traditional Chinese medicine in the perioperative period of ACDF.Methods:patients who underwent single-stage ACDF because of cervical Spondylotic myelopathy from December 2019 to December 2020 in the Spinal Department of Henan Hospital of traditional Chinese Medicine(the second affiliated Hospital of Henan University of traditional Chinese Medicine)were randomly divided into experimental group(n = 29)and control group(n= 30)according to random number table.The control group received routine drug treatment and functional exercise after operation,while the experimental group was given Buyang Huanwu decoction,1 dose per day,in the morning and evening for a total of 7 days.Pain visual analogue(Visual Analogue Scale,VAS)score was used to evaluate the degree of pain before operation,2days after operation and 7 days after operation.Spinal cord function was evaluated by Japanese Orthopaedic Association Spinal Cord State score(Japanese Orthopaedic Association Scores,JOA).Postoperative anemia was evaluated by hemoglobin(Hemoglobin,Hb)level and hematocrit(Hematocrit,Hct).TCM clinical symptom score was used to evaluate the relief of TCM symptoms.The above data were recorded and statistical methods were used to evaluate the curative effect.Results:1.Comparison of baseline data between the two groups: gender data,segmental data,age data,body mass index,operation time,intraoperative blood loss and other aspects(P > 0.05).The data of the two groups are comparable.2.The level of hemoglobin: the results of Hb at each time point between the two groups showed that there was no statistical significance on the 2nd day after operation,while on the 7th day after operation,the HB value of the two groups was basically the same,and on the 7th day after operation,the HB value of the two groups was basically the same,and there was a significant difference between the two groups by t-test on the 7th day after operation.Combined with the analysis of variance of repeated measurement,when only considering the time effect,the Purge0.000 < 0.05,when considering the time-group effect,Preservation 0.007 < 0.05,Buyang Huanwu decoction was significantly better than the control group in relieving postoperative anemia.3.Hematocrit: the results of HCT value at each time point between the two groups were analyzed,and the results showed that there was no statistical significance on the 2nd day after operation,while on the 7th day after operation,the HCT value was 0.006 < 0.05,which was statistically significant,indicating that the HCT values of the two groups were basically the same before and2 days after operation,and there was a significant difference between the two groups by t-test on the 7th day after operation.Combined with the analysis of variance of repeated measurement,when only considering the time effect,the Pure 0.000 < 0.05,when considering the time-group effect,the Preservation 0.006 < 0.05,Buyang Huanwu decoction is significantly better than the control group in relieving postoperative anemia.4.VAS score: the results of VAS score at each time point between the two groups were analyzed,the results showed that there was statistical significance at 2 days after operation(0.016 < 0.05),and there was no statistical significance at 7 days after operation(0.205 > 0.05),indicating that the VAS scores were basically the same before and 7 days after operation,and there were significant differences in VAS scores between the two groups at 2 days after operation.Combined with the analysis of generalized estimation equation: 1.The results of comparison at different time points were statistically significant,and the VAS scores of the two groups changed with time before and after treatment.2.The interaction effect of group * time points showed that P=0.015 < 0.05,indicating that the application of Buyang Huanwu decoction could reduce the VAS score of patients in a shorter time and effectively relieve pain.However,it failed to show obvious advantages in the longer-term comparison.5.JOA score: the results of JOA score at each time point between the two groups were analyzed,the results showed that there was no statistical significance at 2 days after operation,while the postoperative 7 days after operation was 0.042 < 0.05,which was statistically significant,indicating that the JOA scores of the two groups were basically the same at 2 days after operation,and there was a significant difference in JOA scores between the two groups 7 days after operation.Combined with the analysis of repeated measurement variance,when only considering the time effect,we can get the Purge 0.000 < 0.05;when considering the time-group effect,we can get the Purge 0.046 < 0.05,indicating that Buyang Huanwu decoction is better than the control group in improving the spinal cord function after operation.6.TCM clinical symptom score: the results of TCM clinical symptom score before and after treatment between the two groups were analyzed,and the results showed that the clinical symptom score of patients after treatment was 0.006 < 0.05,which was statistically significant,indicating that the improvement of TCM symptoms of postoperative patients treated with Buyang Huanwu decoction was more obvious.Conclusion: Buyang Huanwu decoction can relieve the anemia of postoperative patients,relieve postoperative pain quickly,improve spinal cord function,relieve postoperative symptoms of qi deficiency and blood stasis,be beneficial to the recovery of early postoperative body function and early getting out of bed activity of patients,and achieve the effect of accelerating rehabilitation after ACDF,which is worthy of clinical application. |
