Objective To investigate the application value of CT and B-ultrasound-guided percutaneous lung biopsy and Endobronchial Ultrasound using Guide Sheath(EBUS-GS)in peripheral lung lesions.Methods A retrospective analysis of the clinical date of 361 patients with peripheral lung lesions founded by chest CT from January 2017 to June 2020 in General Hospital of Ningxia Medical University,Cardiovascular and cerebrovascular disease hospital of Ningxia Medical University and Ningxia Fourth People’s Hospital.They were divided into three groups:CT guided percutaneous lung puncture group(n=158),B-ultrasound-guided percutaneous lung puncture group(n=161)and EBUS-GS group(n=42).We compared and analyzed the diagnostic rate,complication rate and related influencing factors of the three group.Results A total of 361 patients were collected,including 158 cases in CT group,161 cases in B-ultrasound group and 42 cases in EBUS-GS group.The diagnostic yield of CT group was 72.8%,that of B-ultrasound group was 68.9%,and that of EBUS-GS group was33.3%.There was statistical difference in the diagnostic yield among the three groups(P <0.05).The diagnostic rate of CT group and B-ultrasound group was higher than that of EBUS-GS group(P<0.05).The incidence of complications was 42.4% in CT group,11.8% in B-ultrasound group,and 7.1% in EBUS-GS group,with statistical difference(P<0.05).Further comparison between the two groups,the incidence of complications in CT group was higher than that in B-ultrasound group and EBUS-GS group(P<0.05).The difference of the size of the lesions(≥3cm and<3cm),the distance between the lesions and the chest wall(<2cm and≥ 2cm)had statistical differences on the diagnosis rate and the incidence of complications among the three groups.Conclusion 1.CT and B-ultrasound-guided percutaneous lung puncture has higher clinical value in the diagnosis of peripheral lung lesions.2.Compared with the other two groups,EBUS-GS group had the lowest incidence of complications.3.The study shows that the clinical application value of EBUS-GS in peripheral pulmonary lesions still needs to be further explored.
